Analysis On Mental Health Status Of A Certain Institution Employees Undergoing Physical Examination

ObjectiveHealth includes physical and mental health, traditional physical examination is centered on physical health checks, not so much on the population's mental health checks. Investigation of mental health status is the first step to mental health examination, which provides first-hand information for improving psychological health management. Investigation of mental health status is a premise for early detection of psychological sub-health and psychological diseases. This study intends to analyze psychological health status of the employees undergoing physical examination and effects of high incidence diseases on psychological health through investigating psychological health status and medical examination results to get a deeper understanding of mental and physical health and provide a basis for improving the physical and mental health management level.MethodsQuestionnaires were distributed to the employees of the certain institution before their physical examination, the questionnaires contained two parts, part one includes the general condition, life style and physical condition of the employees, part two was Symptom Checklist 90(SCL-90). SCL-90 contained 90 entries, each entry used scoring criteria of 1 to 5 points according to the symptoms ranging from asymptomatic, mild, moderate, severe to grievous, and the sum of 90 entries was defined total score. Different entries represented 10 kinds of psychological sensitive dimensions: body dimension, force dimension, interpersonal sensitivity dimension, depression dimension, anxiety dimension, hostility dimension, horror dimension, paranoia dimension, spiritual dimension and other dimension. The sum of scores in the same dimension was the dimension score. Then physical examination was performed, specific projects were uniform which contained routine blood and urine, liver function, blood glucose, blood lipid, renal function, blood pressure, electrocardiograph, hepatobiliary, pancreatic, splenic, renal and thyroid color doppler ultrasound, arteriosclerosis detection, bone mineral density test, anteroposterior and lateral position chest radiographs and so on. The comorbidity statuses of the top five high incidence diseases were investigated. The general condition, life style and physical condition of all employees, the comorbidity of the top five high incidence diseases and mental health statuses results were described and analyzed.ResultsThe general condition: 500 cases, which were eligible for inclusion standards, were included in this study, 215 males(43% of the total cases) and 285 females(57% of the total cases); the age range was 24- to 60-year-old, the average age was 44.17±8.10-year-old; the proportions of employees of female, aged between 40 and 49 years old, Han, intellectuals, married status, bachelor degree and above, no religion and no stress events last week were the most. The life style: the proportions of employees with non-smoking groups and sedentary time greater than or equal to 6 hours and less than 8 hours group were most. The physical condition: in no chronic diseases history, no obvious discomfort and taking no health products or drug groups there were more cases.The top five high incidence diseases from the 2016 physical examination results were successively thyroid nodules(55.4%), cholecystitis(42.6%), fatty liver(41%), reduced bone density(37.2%), atherosclerosis(33.4%). According to the comorbidity status based on top five high incidence diseases, the employees were divided into the following six groups, including no disease, one, two, three, four and five of the top five high incidence diseases. The employees suffering from two kinds of high incidence of disease, with the number of 123 people, account for the most part- about 24.6% of the total number.The median of 500 workers' SCL-90 scores was 100, minimum was 90, highest was 207, range was 117. Diseases were leveled as 1 to 5 points in accordance with symptoms ranging from asymptomatic, mild, moderate, severe to grievous, and the results showed asymptomatic and mild level employees made up the greater part.The general condition distribution results of the mental health status in 500 employees(SCL-90 results): differences of scores of employees of different gender were not statistically significant in the 10 dimensions respectively and total 10 dimensions; the scores distribution differences among different age groups were statistically significant in body dimension(P<0.05); different ethnics had statistical significant differences in terror dimension(P<0.05); employees of different occupations and different marital status had no statistically significant differences in the 10 dimensions scores respectively and total 10 dimension scores; the employees of different cultural levels had different scores in body dimension, force dimension, interpersonal sensitivity dimension, depression dimension, anxiety dimension, hostility dimension, spiritual dimension, other dimension and the total 10 dimension scores(P<0.05); employees of different religious beliefs had differences in force dimension, interpersonal sensitivity dimension, depression dimension, spiritual dimension(P<0.05); employees in different life stress events last week groups got significantly different scores in 10 dimensions respectively and total 10 dimension(P<0.05).The life style distribution results of the mental health status in 500 employees: employees in different sitting time groups were statistically different in body dimension, force dimension, depression dimension and other dimension(P<0.05); there was a statistical significance of mental health differences among different smoking groups in interpersonal sensitivity dimension, hostility dimension and spiritual dimension(P<0.05).The physical condition distribution results of the mental health status in 500 employees: the employees with different chronic disease status had different scores in body dimension, force dimension, depression dimension, hostility dimension, spiritual dimension, other dimension and total 10 dimension scores(P<0.05); employees of different discomfort status had different scores in 10 dimensions respectively and total 10 dimension scores(P<0.05); the mental health status of different drug-use employees differed in body dimension, force dimension, interpersonal sensitivity dimension, depression dimension, anxiety dimension, hostility dimension, paranoia dimension, spiritual dimension, other dimension and the total 10 dimension scores(P<0.05).The high incidence diseases and comorbidity status of the mental health status in 500 employees: There were significantly differences in 10 dimensions, respectively and total 10 dimensions scores among six groups.ConclusionThe following conclusions were drawn by mental health status survey and physical examination of a certain institution 500 employees undergoing physical examination:1. The general conditions of the employees including age, nationality, culture, religion, stress events last week had different effects on mental health status of the certain institution employees undergoing physical examination.2. The life style containing sedentary time and smoking had different effects on mental health status of the certain institution employees undergoing physical examination.3. Different physical condition including history of chronic diseases, perceived discomfort and medication condition had different effects on mental health status of the certain institution employees undergoing physical examination.4. The different status of comorbidity of the top 5 high incidence diseases detected in physical examination, including thyroid nodules, cholecystitis, fatty liver, bone density reduction and arteriosclerosis had different effects on the mental health status of the certain institution employees undergoing physical examination.
