Background In recent years,with the rapid economic development,the changes of the social environment and the lifestyle of the residents,the disease and death spectrum of residents have also changed.The deaths caused by cardiovascular diseases,cerebrovascular diseases,diabetes,malignant tumor and other chronic non communicable diseases have accounted for an increasing proportion.As a basic data for the rational allocation of health resources,the data of death surveillance has a very important effect on the evaluation of the overall health of the residents in a certain area.The system analysis of cause of death data is one of the important contents reflecting the health level of residents,which can achieve the death level,death causes and trends of the residents in an area,so as to determine the priorities for the disease prevention and control work,and to provide a scientific basis for the formulation of health policies and measures.The Dezhou city launched a pilot work of disease surveillance based on the cause of death surveillance in Wucheng County in 2005,which was a part of the Shandong provincial disease surveillance network.By 2011,the death surveillance work in Dezhou city had extended to the city’s 11 counties.From the beginning of 2012,the Dezhou Municipal Center for disease control had strengthened the training and supervision of death surveillance work in counties,to ensure that the quality and quantity of death surveillance data was increasing year by year.The study analyzed the death surveillance data in Dezhou City,Shandong province from 2011 to 2015,to understand the main diseases that affect the health and life of residents in Dezhou city and the degree of the harm,and to look for the main health problems affecting the health of residents in Dezhou city,which can provide reference and basis for health administrative departments to make specific policies.Objective To understand the death level,death causes and trends of the residents in Dezhou city from 2011 to 2015,and to look for the main health problems affecting the health of residents in Dezhou city,so as to provide a scientific basis for targeted disease prevention and control strategies.Materials and methods According to the date of death from January 1st to December 31st,the registered address of Dezhou city,audit and other conditions,the data 2011,2012,2013,2014,2015 were derived from the death registration reporting system of China Center for Disease Control and prevention.The population data were from the National Bureau of statistics.The international classification of diseases ICD-10 was used to encode the cause of death.The 2010 national census data were used to calculate the standardized mortality rate.The data were collected and analyzed by using Excel2007 and SPSS 18.0 software.The descriptive statistics method was used to calculate the crude mortality,standardized mortality rate,constituent ratio of death,rank of death cause and other indicators.Results The total population of Dezhou City from 2011 to 2015 was 28330982,of which 14385561 were male,13945421 were female.The sex ratio was 1.03:1.Compared with the 2000 census data in Shandong Province,children’s burden coefficient decreased significantly,while elderly burden coefficient increased.Through the statistical analysis of the death cause data in five years,it was concluded that(1)The crude mortality rate of the total population of Dezhou City from 2011 to 2015 was 610.62/100 thousand,and the standardized mortality rate was 428.10/100 thousand.Compared with women,both the crude mortality or standardized mortality of men are higher;(2)The top five major causes of death of residents in Dezhou city from 2011 to 2015,were cerebrovascular diseases(the crude mortality rate:363.70/100 thousand,accounting for 59.56%),malignant tumor(crude mortality rate:115.76/100 thousand,accounting for 18.96%),injury and poisoning(the crude mortality rate:36.85/100 thousand,accounting for 6.04%),respiratory diseases(the crude mortality rate:28.09/100 thousand,accounting for 4.60%)and digestive system diseases(the crude mortality rate:12.06/100 thousand,accounting for 1.98%).The top five causes of death accounted for 91.14%of total death.(3)In the death caused by malignant tumor in Dezhou city from 2011 to 2015,the top five rank of death cause were lung cancer(the crude mortality rate:22.41/100 thousand),gastric cancer(the crude mortality rate:17.47/100 thousand),liver cancer(the crude mortality rate:12.01/100 thousand),esophageal cancer(the crude mortality rate:7.87/100 thousand)and colorectal cancer(the crude mortality rate:4.04/100 thousand);(4)The average life expectancy in Dezhou City from 2011 to 2015 was 78.45 years old,of which the male was 76.13 years old,and the female was 80.94 years old.Conclusions and suggestions The results showed that the standardized mortality rate of the total population of Dezhou City was higher than the results of the third National Death survey,counting down in the counties of the whole province.There were still a large number of missing death in the surveillance work.The deaths caused by chronic diseases accounted for 89.74%of the total deaths,which had become the main risk factors of death of the total population of Dezhou City.Due to the factors like the regional environment,life style and habits of people,and the proportion of the population,compared with the results in Shandong Province in the same period,the proportion of three kinds still had a slight difference.The top rank of death cause had certain differences with that of the province and other cities in the province,which also reflected the rank of death cause had the characteristics of regional difference.Suggestions:(1)Strengthen public awareness of the major risk factors for chronic non communicable diseases,advocate the limit of smoking and alcohol,healthy diet,appropriate physical exercise and healthy lifestyle.(2)Control the disease as sooner as possible,deeply study the environmental and social factors of the cardiovascular and cerebrovascular diseases,malignant tumors,injury and poisoning,and dynamically monitor the risk factors.(3)Carry out the basic public health programs,establish and improve the residents’ health records,carry out the regular physical examination of the elderly and follow up the patients with hypertension and diabetes regularly.(4)Further strengthen the death surveillance work of Dezhou city,reduce the false negative rate,and improve the quality of reports.
