Clinical Observation Of Jiawei Wulingsan On Patients With Nephrotic Syndrome And Tubular Injury

Objective:To observe the effect of Jiawei wulingsan grannule on edema and electrolyte metabolism and function of tubular in patients with primary nephrotic syndrome and differentiation of TCM of yang deficiency of both spleen and kidney,and try to explore its mechanism.Methods:70 patients with PNS and differentiation of TCM is yang deficiency of both spleen and kidney were randomly divided into two groups by 35 cases in each.Cases in control group were given western medicine only which according to standard protocol.And the trial group were given Jiawei wulingsan grannule combined with western medicine.The course of treatment is 2 weeks.During the treatment,daily weight and urinary volume of patients were observed,and edema were evaluated.Meanwhile plasma albumin,urinary protein,electrolyte(serum sodium and kalium)and markers of tubular injury before and after treatment were detected respectively.Results:l.The symptom of edema and yang deficiency of both spleen and kidney of cases in two groups was improved,but the effect of trial group was better than the other one.2.Compared the before and after treatment,there were significant differences in plasma albumin and urinary protein quantitative analysis of two groups,and the effect of increasing plasma albumiin and reducing urinary protein of trial group was better than the other one.3.Compared the before and after treatment,there were significant differences in disorder of electrolyte of two groups,and the incidence of disorder electrolyte of trial group was lower than the other one.4.Compared the before and after treatment,the markers of tubular injury all declined,there were significant differences in NAG and NGAL exceptβ2-MG of two groups,and the level of decline of trial group was higher than the other one.Conclusion:The Jiawei wulingsan granule could decline the level of tubular injury during using western medicine for diuretic treatment,and reduce the occurrence of disorder of electrolyte,and improve the clinical symptom of patients with PNS and yang deficiency of both spleen and kidney on the whole.There were no any side effects were found during the treatment of the Jiawei wulingsan.
