The Retrospective Research On The Ankylosing Spondylitis Cases Treating By Professor Zhou Zhongying Based On Data Mining

Objective:To summarize Professor Zhou Zhongying s academic idea and clinical experience in diagnosing and treating ankylosing spondylitis,in order to further improve the clinical efficacy of the traditional Chinese medicine in ankylosing spondylitis on the basis of his clinical practice for dozens of years and modern data mining technology.Methods:All ankylosing spondylitis cases from Professor Zhou Zhongying clinic in recent ten years were collected.According to inclusion and exclusion criteria,58 cases with 196 visits were picked up and established an ankylosing spondylitis database.In order to find out the rules of clinical manifestation,pathogenesis,therapy and medication,data mining technology like frequencies,factor analysis and association analysis were used.The analysis results were discussed and summarized combined with Professor Zhou Zhongying s clinical experience.Results:All 58 cases of ankylosing spondylitis,42 are male and 16 are female,the age is 20-68,the average age is 37.03.According to the results of frequency analysis,symptoms frequencyappear higher with lower back pain,fear of cold,limited mobility,sacroiliac and hip pain,and joint stiffness account for a higher percentage(63.78%,37.24%,28.57%,27.04%,25.51%respectively).Pathogenesis such as kidney Du meridian deficiency,blockage of phlegm and blood stasis,liver and kidney deficiency,blockage of wind-damp,internal accumulation of damp-heat,blockage of cold-damp take up a higher percentage(49.49%,49.49%,33.16%,31.12%,13.78%,12.24%respectively).A wide range of herbs were used in the treatment of ankylosing spondylitis,199 kinds of herbs were used in all of the 58 cases.According to the frequency of usage,the top ten herbs are Xuduan,Xianlingpi,Tubiechong,Zhinanxing,Qiannianjian,Bajitian,Gusuibu,Zhiquanxie,Shudihuang,Danggui,which take up 79.08%,67.86%,63.78%,62.24%,60.71%,58.16%,57.65%,51.53%,49.49%,43.88%.Conclusion:This study demonstrated professor Zhou Zhongying’s ideas and characteristics of syndrome differentiation and treatment in ankylosing spondylitis according to the analysis of data mining results.On the one hand,wind,cold,dampness,heat,phlegm and blood stasis are the main causes of ankylosing spondylitis.Pathogenesis are mainly Du meridian deficiency,blockage of phlegm and blood stasis,liver and kidney deficiency,blockage of wind-damp,internal accumulation of damp-heat.On the other hand,Strengthening vital Qi and replenishing the deficiency,dispelling wind and dampness are the basic treatment of ankylosing spondylitis.Tonics should be used throughout the course of disease.The common-used herbs include Xuduan,Sangjisheng,Xianlingpi,Buguzhi,Bajitian,Shudihuang,Duzhong,Qiannianjian,Gouji,etc.The characteristics in treatment of ankylosing spondylitis by professor Zhou Zhongying are:prescription according to the position of disease;combination of both syndrome differentiation and disease differentiation;reasonable application of herbs including insects;cautious usage of toxic herbs.
