Objective:To observe the clinical effect of Chang Fu An Decoction in treating diarrhea type Irritable Bowel Syndrome(IBS-D),through comparing the symptomatic quantization,assessing symptoms improvement and overall effect;the proximal gastric volume of IBS-D patients were detected by ultrasound before and after treament,observing the proximal gastric motor function,to prove disorders of proximal stomach movement in the IBS-D.To investigate the impact of the TCM on the function of the proximal stomach through the comparision with the control group of traditional Chinese medicine treatment effect before and after treatment.Method:48 eligible cases were randomly divided into the treatment group and the control group,24 cases in each group,were treated with the Bifid Lriple Viable,and the treatment group were given the Chang Fu An Decoction besides probiotics.Both group were treated for a couse of 6 weeks.The symptom scores were tested before treatment and at 2nd week,4th week,and 6th week after treatment.The proximal stomach volume of two groups of patients were detected with ultrasound before and after treatment as the following steps.After at least 8 hours on an empty stomach,inspectors drink warm water 500ml,and the proximal gastric volume were measured immediately for V1;30mins later measured again for V2.The Emptying Fraction of upper gastric could be calculated by the formula:EF=(VI-V2)/V1*100%.The clinical efficacy and the changes of volume and emptying fraction of proximal stomach were observed.Another 20 healthy subjects were selected as reference standard,whose stomach volume and EF were acquired throgh method above.Results:48 cases were enrolled in the study in total.2 cases of control group loss to follow-up for far from home in the process of research.1 case of treatment group stoped the research because of working on a business trip abroad.Finally,a total of 45 patients finished this research,and the data was statistical analyzed.The clinical results showed:The total efficative rate and markedly effective rate was 91.3%and 69.57%in the treatment group.While the total efficative rate and markedly effective rate was 72.72%and 27.27%in the control group.The total effectiveness of treatment group is better than that of control group with a obvious differences between the two groups(P<0.05);The symptoms of both groups showed obvious improvement than before the treatment,the difference was statistically significant(P<0.01),and the treament group better than the control group with a significant difference(P<0.01).In the five symptoms such as abdominal pain,diarrhea,abdominal distension,thoughtfulness and nervousness,dyspepsia,the treatment group scored less than the control group with statistically significant difference(P<0.05).The results of the proximal stomach movement indicated:The emptying fraction of both groups were scored significantly higher than the healthy group(P<0.05);but the differences of both groups after the treament and healthy group were not statiscally significant(P>0.05).The comparison of the results after treament and before treament was not significantly different(P>0.05),nor was it among groups(P>0.05).Conclusion:Chang Fu An could significantly improve the clinical curative effect in the treatment of IBS-D,showing good effect on the clinical symptoms and mood disorders,which illustrats TCM is a effective treament of IBS.The proximal stomach disfunction was observed by the mothod of ultrasonography and the emptying fraction declined after treament,though no diference between two groups.The mechanism of TCM treament still need to be investigated further.
