Objective This study through the observation of non dialysis chronic kidney disease mineral and bone metabolism(CKD-MBD)and syndrome differentiation of fibroblasts in serum of patients with kidney deficiency and damp heat in the growth factor(FGF23)levels,as well as Rong Huang tonify kidney detoxification granules(short for Rong Huang granule)on intervention effect,for exploring the role of Rong Huang granule on CKD-MBD in patients with kidney deficiency and damp heat and its possible mechanism.Methods 66 patients who met the inclusion criteria were randomly divided into treatment group(n= 33)and control group(n = 33).In the course of treatment,the patients were treated in the treatment group(n = 4),and the final treatment group(n = 32),the control group(n = 30),and the normal group(n = 20).Two groups of patients were given low salt,low fat,high quality and low protein diet,correct anemia and calcium and phosphorus metabolism,inhibition of hyperparathyroidism,blood pressure control and other basic treatment,and rhubarb enema retention enema.The treatment group was added with yellow particles with warm water,a bag / time,three times / day.The two groups were treated for 8 weeks.Observe the symptom changes before and after treatment,the two groups of patients during treatment and the level of renal function and possible adverse reactions;serum calcium before and after treatment and monitoring during treatment in two groups(Ca),phosphorus(P),parathyroid hormone(i PTH),alkaline phosphatase(ALP)and FGF23 levels the situation;at the same time to detect the level of serum FGF23 in our hospital physical examination center in healthy group,and the serum FGF23 levels of two groups of patients before treatment and compared with each other.According to the results to determine the efficacy,and statistical processing.Results After treatment,the treatment group of TCM syndrome curative effect,the clinical efficacy of the disease and the improvement of the main symptoms were significantly better than the control group,the difference was statistically significant(P<0.05 or P <0.01).After treatment,the TCM Syndromes of two groups of patients were with the extension of treatment and was significantly lower(P <0.05 or P <0.01);compare the efficacy of two groups at the same time point in the course of treatment,the efficacy of the treatment group at each time point were significantly better than the control group(P<0.01).After treatment,the indexes of renal function in the treatment group were significantly improved(P <0.05 or P<0.01);the serum BUN of the control group was significantly improved(P <0.05),while the other indexes were not significantly improved(P>0.05).After treatment,the serum Ca,P,i PTH,and ALP in the treatment group were significantly improved(P<0.05 or 0.01)compared with those before treatment.The control group had no significant improvement(P<0.01)except for i PTH(P>0.05).Before treatment,the serum FGF23 levels of the two groups were significantly higher than those of the normal group(P<0.01),but there was no difference between the two groups(P>0.05).After treatment,the serum FGF23 level in the treatment group was significantly improved,the difference was statistically significant(P<0.05);the control group had no significant improvement in serum FGF23,and the difference was not statistically significant(P>0.05).The Pearson analysis showed that the serum FGF23 level was significantly negatively correlated with e GFR and positively correlated with i PTH and ALP,and the difference was statistically significant(P<0.01).In the course of treatment,no adverse reactions occurred in the two groups.Conclusions The level of serum FGF23 in patients with non dialysis CKD-MBD kidney deficiency and damp heat syndrome was significantly higher than that of healthy people,which indicated that there was a high expression level of serum FGF23 in these patients.Rong Huang granule can improve the curative effect,TCM syndrome of non dialysis CKD-MBD patients with kidney deficiency and damp heat disease clinical curative effect and the main symptoms and protect residual renal function,elevated serum calcium,serum phosphorus and reduce the level of i PTH,so as to achieve the purpose of preventing CKD-MBD,reduce its mechanism and the level of serum FGF23 may have a close relationship. |
