Objective:1)To establish a database of high-risk population of stroke,including basic information,risk factors and follow-up information of high-risk groups according to the cross-sectional survey of residents aged 40 years and over in the Jiangling community.2)To analyze the demographic characteristics and distribution characteristics of the risk factors associated stroke,including hypertension,dislipidemia,diabetes,atrial fibrillation,smoking,apparently overweight or obese,lack of exercise and positive family history of stroke in Jiangling community of high-risk population aged 40 years and over.3)To investigate the distribution of the main risk factors of the high risk population aged 40 years and over in Jiangling community,and the distribution of the main risk factors depending on different gender and age.4)To explore the prevalence rate of stroke risk factors in the population exposed aged 40 years and over,and analyze the characteristics of stroke in Jiangling community inhabitants when compared with the national population.Methods:This study was supported by the special fund for public welfare of National Health and Family Planning Commission in 2014,named “Screening and intervention program for population at high risk of stroke”.Screening of stroke for residents aged40 years and older using the cross-sectional method in Jiangling community.The data were analyzed statistically by SPSS23.0 software.Result:The result of the epidemiological investigation is that about 404 high-risk groups of stroke were screened from the 6073 effective questionnaires in Jiangling community of the Jiangxi Province,Nanchang City.Part1 The general situation of the respondentsThere were 229(56.68%)male and 175 females(43.32%)in this survey,and the male to female ratio: 1.30:1.The aged of the survey population is from 40 to 93 years,the average age of total is(63.93±9.74)years old,the average height is(163.82±7.84)cm,the average weight is(67.31±12.15)kg,the average BMI is(24.93±3.51).There were 92 people(22.77%)in the age group of 60-64 years,of which there were 55males(13.61%)and females(37)(9.16%).The marital status was 355(87.9%).268 people in low education group accounted for 66.34%,the number of highly educated people were 268 which accounted for 33.66%.The number of professional and technical personnel was 119(29.46%)and workers was 163(40.34%).The average monthly expenditure on medical expenses was 335(82.92%).There was 391(96.78%)people who use medical insurance to pay for medical carePart2 Analysis of the major risk factors for the high-risk groups of stroke1)The top three of the high risk factors were hypertension(40.34%),dyslipidemia(40.34%)and smoking(40.34%)in the high risk population aged 40 years and over in Jiangling community.Among the top three risk factors of male were hypertension,smoking and dyslipidemia,and the three major risk factors of female were hypertension,dyslipidemia and obesity.2)Comparison of risk factors of stroke among high risk population in different gender,there were more females with hypertension,atrial fibrillation,lack of exercise and light physical labor,more males with smoking and the distribution of dyslipidemia,diabetes,significantly overweight or obesity,family history of stroke,TIA were basically equal in the male and female.3)Comparison of risk factors of stroke among high risk population in different age,hypertension and dyslipidemia are two major risk factors in the high risk population of stroke patients aged 40 years and over.The risk factors of the exposure rate were different in different age groups.4)Comparison of morbidity rate in exposed population with different high risk factor of stroke,the populations of top morbidity rate of stroke were groups of lack of exercise or light physical labor(32.35%),atrial fibrillation(29.55%),diabetes(25.00%).There were also gender differences.Part 3 Compared with the national level1)Comparison of risk factors distribution among high risk population in stroke.The detection rate of positive risk factors of hypertension,diabetes mellitus,smoking and family history of stroke was higher than that of national average.Jiangling community and the country have great similarity in the gender distribution.The hypertension,dyslipidemia,atrial fibrillation,lack of exercise or light physical labor,family history positive of stroke and TIA are higher in women,but smoking is higher in men.2)Comparison of prevalence of stroke in exposed stroke patients with different stroke risk factors.The prevalence rate of risk factors(including hypertension,diabetes,smoking,family history positive of stroke)is higher than the national,however,the order was consistent with the national average,followed by positive family history of stroke,atrial fibrillation,diabetes,high blood pressure,lack of population,significantly overweight or obese people,dyslipidemia and smokers.Conclusion:(1)The two major risk factors were hypertension and dyslipidemia in the high risk population aged 40 years and older and the distribution of risk factors were depending on gender and age.(2)The prevalence of the risk factors of stroke risk factors among people over the age of 40 years in Jiangling community were lack of exercise or light physical labor,atrial fibrillation,and diabetes.(3)The prevalence of positive risk factors of hypertension,diabetes mellitus,smoking and stroke was higher than those of the national average,and the prevalence of stroke was significantly higher than those of the national level in the high risk group of stroke patients aged 40 years and older.(4)The risk factors of high-risk population of stroke were not under control in Jiangling community,and the primary and secondary prevention of stroke should be strengthen.
