Objective: Study the influence of different vaginal douche methods on vaginal microecosystem and the comfort degree of patients to search for a better vaginal douche method,as to provide theoretical basis for clinical nursing work.Methods: 180 gynecological benign tumor patients were chosen for the study from the gynaecology department of a Class ? Grade?hospital in Hebei Province from December,2015 to October,2016,who were randomly divided into six groups(experiment group 1 to experiment group 6)by using random function of EXCEL Spreadsheet.The vaginal secretion of patients were collected after they were admitted to hospital without doing any vaginal examination.According to the examination results,three different concentrations of Benzalkonium Chloride with two douche methods,which make it total six different methods,were applied to patients whose vaginal flora density is between III to VI grade.For the two douche methods,one is rising vulva first,then opening the labia minora with left hand,inserting the head-end of douche slowly into vagina from the side wall to reach the vaginal posterior fornix and irrigating from all directions with gently moving the head-end of douche around cervix;the other is rising vulva first,then opening the labia minora with left hand,exposing the cervix with speculum followed by inserting the head-end of douche into the vagina to irrigate and turning the speculum around at the same time.The three different concentration of Benzalkonium chloride is: 0.02%,0.035% and 0.05%.The bacteria density and pH value were tested respectively after collecting the patient's vaginal secretion before irrigation and 24 hours later after irrigation.Collect data of the bacteria density by Glen Staining and pH value bypH precision test paper(Three Ace Reagent Co.,LTD.Shanghai)before irrigation and 24 hours later after irrigation,and the comfort degree of the six group patients evaluating on the basis of the visual analogue scale(VAS)method.The bacteria density and pH value data were compared within and between the six groups.The comfort degree data were compared between the groups.SPSS19.0 Statistical Software was used for data analysis.The influence of the six different vaginal douche methods on bacteria density,pH value and the comfort degree were discussed.Results:1 Basic characteristics of six group patients.Patients in six groups in age(P=0.905),vaginal cleanness no statistical differences(P = 0.991),etc.2 Comparing density of vaginal flora of six group patients within the groups before irrigation and 24 hours later after irrigation.Vaginal bacteria density of experiment group 1(P < 0.001),vaginal bacteria density of experiment group 2(P<0.001),vaginal bacteria density of experiment group 3(<0.0010),vaginal bacteria density of experiment group 4(P <0.001),vaginal bacteria density of experiment group 5(P<0.001)and vaginal bacteria density of experiment group6(P<0.001)are all statistically significant.3 Comparing density of vaginal flora of six group patients between the groups before irrigation and 24 hours later after irrigation.Before vaginal douche,There is no statistical difference(P = 0.670)of the vaginal flora density of six groups patients between groups,there is neither differences(P = 0.294)24 hours later after vaginal douche.4 Comparing vaginal pH values of six group patients within the groups before irrigation and 24 hours later after irrigation.There is no statistical difference of experiment group 1(P=0.066),experiment group 2(P=0.117),experiment group 3(P =0.211),experiment group 4(P=0.088),experiment group 5(P=0.150)and experiment group 6(P=0.142).5 Comparing vaginal pH values of patients whose vaginal pH value 3.8 to 4.5before vaginal douche between the groups before irrigation and 24 hours later after irrigation.Before vaginal douche,There is no difference(P = 0.782)of vaginal pH value of six groups patients between groups,there is neither difference(P =0.456)after vaginal douche.6 Comparing the comfort degree of patients' vaginal douche between the groups.6.1 The comparison of vaginal douche comfort degree between experiment group 1,experiment group 3 and experiment group 5There were no statistical difference in Experiment group 1 and experiment group 3(P =0.822),experiment group 1 and experiment group 5(P=1.000),experiment group 3 and experiment group 5(P = 0.822).6.2 The comparison of vaginal douche comfort degree between experiment group 2,experiment group 4 and experiment group 6.There were no statistical difference in Experiment group 2 and experiment group 4(P =0.261),experiment group 2 and experiment group 6(P=0.339),experiment group 4 and experiment group 6(P = 0.866).6.3 The comparison of vaginal douche comfort degree between experiment group 1 and experiment group 2,experiment group 4,experiment group 6.Experiment group 1 and experiment group 2(P =0.001),experiment group 1 and experiment group 4(P = 0.019),experiment group 1 and experiment group 6(P = 0.012),are statistically significant.6.4 The comparison of vaginal douche comfort degree between experiment group 3 and experiment group 2,experiment group 4,experiment group 6.Experiment group 3 and experiment group 2(P<0.001),experiment group 3 and experiment group 4(P=0.010),experiment group3 and exper iment group 6(P=0.006),are statistically significant.6.5 The comparison of vaginal douche comfort degree between experiment group 5 and experiment group 2,experiment group 4,experiment group 6.Experiment group 5 and experiment group 2(P=0.001),experiment group5 and experiment group 4(P =0.019),experiment group5 and experiment group 6(P = 0.012),are statistically significant.6.6 The comparison of vaginal douche comfort degree between experiment group 2 and experiment group1,experiment group 3,experiment group 5.Experiment group 2 and experiment group 1(P=0.001),experiment group2 and experiment group 3(P < 0.001),experiment group2 and experiment group 5(P = 0.001),are statistically significant.6.7 The comparison of vaginal douche comfort degree between experiment group 4 and experiment group1,experiment group 3,experiment group 5.Experiment group 4 and experiment group 1(P=0.019),experiment group4 and experiment group 3(P = 0.010),experiment group4 and experiment group 5(P = 0.019),are statistically significant.6.8 The comparison of vaginal douche comfort degree between experiment group 6 and experiment group1,experiment group 3,experiment group 5.Experiment group 6 and experiment group 1(P=0.012),experiment group6 and experiment group 3(P = 0.006),experiment group 6 and experiment group 5(P = 0.012),are statistically significant.Conclusion:Vaginal douche can reduce the density of vaginal flora and change vaginal micro-ecological environment.There's no difference of the effect of the two douche methods on the bacteria density and pH value,neit-her do the three concentrations of Benzalkonium Chloride.Benzalkonium Chloride is a safe and excitant small disinfectant.Patients feel more c-omfortable when vagina was washed directly without vagina cervix beingopened by speculum.The study showed that directly washing vagina with disposable vagina douche is preferred from the perspective of the patient comfort,which could improve the patient compliance and nurses work efficiency.
