Therapeutic Effect Of Surgical Treatment On Coony Ⅱ Type Distal Radius Fracture As Compared With Conservative Treatment

Objective To compared and analyze the therapeutic effect of surgical treatment on Coony Ⅱ type distal radius fracture as compared with conservative treatment,so as to provide evidence for the diagnosis and treatment of extra-articular distal radius fracture.Methods This prospective clinical controlled trial was made on 106 patients with Coony Ⅱ type distal radius fracture diagnosed and treated in our hospital from October 2014 to April 2016.In accordance with the different treatment methods,the patients were divided into 2 groups: the surgical treatment group(the observation group)and the conservative treatment group(the control group).According to GW score and PRWE score in the treatment of 6 weeks,3 months,6 months and 1 years of follow-up,the patients were lateral X-ray examination,measurement in patients with palmar tilt,ulnar deviation and other related data,in order to bowl pain was observed in the 2 groups after treatment of wrist joint function and quality of life of patients,and then the recorded data were statistically analyzed,respectively evaluate the clinical curative effect of open reduction and internal fixation and external fixation for the treatment of manipulative reduction.Results 106 cases were all followed up for 1 years,X-ray showed all fractures healed.All patients receive surgery group anatomically,there is one case of Complex regional pain syndrome;The conservative group l0 malunion,3 patients due to other reasons failed to loose splint to the hospital review,caused severe fracture deformities,poor function.Operative treatment had a better effect than conservative treatment according to the Gartland-Werley score and PRWE score at the time point 1.5th,3th,6th months and this superiority was confirmed by statistically significant difference.But the superiority became less and less apparent during the follow-up.There was no statistically significant difference at 12 th month follow-up.Conclusions 106 cases were all followed up for 1 years,X-ray showed all fractures healed.All patients receive surgery group anatomically,there is one case of Sudeck atrophy of bone;The conservative group l0 malunion,3 patients due to other reasons failed to loose splint to the hospital review,caused severe fracture deformities,poor function.Operative treatment had a better effect than conservative treatment according to the Gartland-Werley score and PRWE score at the time point 1.5th,3th,6th months and this superiority was confirmed by statistically significant difference.But the superiority became less and less apparent during the follow-up.There was no statistically significant difference at 12 th month follow-up.Significance Through the study of this clinical trial,we hope to contribute to the research in related fields,and to provide some references for the treatment of distal radius fractures.
