Objective: By comparing the clinical effects of manual reduction and percutaneous pinning internal fixation and open reduction and plate internal fixation in the treatment of elderly C1 and C2 distal radius fractures,in order to provide a reference for the clinical treatment of distal radius fractures.Methods: 80 elderly patients with C1 and C2 distal radius who were treated in the Department of Orthopedics of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine from October 2018 to December 2020 were selected according to the criteria of this study,and were randomly divided into manual reduction percutaneous pinning internal fixation There were 40 cases in each group(pinning group)and open reduction plate internal fixation group(plate group),and they were treated with corresponding treatment methods.Clinical follow-up of all patients was performed to compare the operation time,fracture healing time and complications of the two groups.The palm inclination,ulnar deflection,and radius height of the two groups were measured before and immediately after the fracture healing.At 3 months,6 months,and 12 months after surgery,the modified Gartland-Werley score method was used to compare the function of the affected limbs of the two groups.Results: All 80 patients included in this study were clinically followed up for 12 months.The operation time of the pinning group was(53.6±11.4)min,and the operation time of the plate group was(73.4±10.7)min.The difference between the two groups was statistically significant(P=0.000<0.05).The fracture healing time of the pinning group was(10.25±1.26)weeks,and the fracture healing time of the plate group was(11.13±1.24)weeks.The difference between the two groups was statistically significant(P=0.002<0.05).There were 2 cases of superficial needle tract infection in the pinning group,1 case of reflex sympathetic dystrophy due to Kirschner wire stimulation,1 case of large thenar numbness in the plate group,1 case of flexor tendonosynovitis of the fingers.Comparing the complications of the two groups,The difference was not statistically significant(P=0.644>0.05).There were no statistically significant differences in the palmar inclination angle,ulnar angle and radius height before and immediately after the fracture healing(P>0.05).The modified Gartland-Werley score of the plate group was better than that of the pinning group at 3 months after surgery(P<0.05),but there was no statistically significant difference in the functional scores between the two groups at 6months and 12 months after surgery(P>0.05).Conclusion: For elderly C1 and C2 distal radius fractures,manual reduction and percutaneous pinning internal fixation has good clinical efficacy and safety,and it has the advantages of simple operation,short operation time,small trauma,and short fracture healing time,which is worthy of clinical practice Promote use.
