Objective:This essay is to make a comparative study of conventional ultrasound examination,Automated Breast Volume Scanner(ABVS)and Mammography(MG)in the diagnosis of breast neoplasm and to evaluate the value of ABVS to the diagnosis of breast neoplasm.Methods:From May to December in 2016,120 female patients with a total of 156 lines of breast neoplasm were diagnosed through conventional ultrasound,ABVS and MG.All the pathologic results of puncture and postoperation have been accessed.Firstly,the conventional ultrasound was adopted to scan bilateral breasts,with the size,shape,border,internal echo,calcification and decrescence of the lesion recorded as well as rated,according to BI-RADS.Then ABVS was implemented to scan bilateral breasts in full-volume,with the images analyzed as well as rated,according to BI-RADS.With the pathologic results as the gold standard and the results from MG,the diagnostic efficiency of conventional ultrasound,ABVS and MG was comparatively analyzed.During this,X2 is used to measure and P<0.05 is considered as of significance.Z-testing is used to compare the areas under the curve of the conventional ultrasound,ABVS and MG.Kappa is used to evaluate the consistency of calcification of ABVS and MG.Results:(1)The chosen 120 women were aged between 23 and 78(Averaged as 47.66 ±12.38).The patients with benign lesions were aged between(40.75±11.44).The patients with malignant lesions were aged between(52.77±11.42).The difference is statistically significant(t=5.977,P<0.05).(2)The diameter of 156 tumors of breast measured between0.4 cm and 6.1 cm(Averaged as 1.78 ± 1.03 cm).Those of benign lesions were(1.79+-1.16),while those of malignant lesions were(1.76±0.9).The difference has no significance(P=0.873).(3)66 malignant lesions were firmly diagnosed by conventional ultrasound,while the actual number was 83(79%).53 benign lesions were diagnosed by conventional ultrasound,while the actual number was 73(72%).The sensitivity of conventional ultrasound in the diagnosis of breast neoplasm was 79.5%,as well as 72.6%for specificity,and 76.3% for accuracy.81 malignant lesions were firmly diagnosed by conventional ultrasound,while the actual number was 83(97%).64 benign lesions were diagnosed by conventional ultrasound,while the actual number was 73(87%).The sensitivity of ABVS in the diagnosis of breast neoplasm was 97.6%,as well as 87.7% for specificity and 92.9% for accuracy.67 malignant lesions were firmly diagnosed by conventional ultrasound,while the actual number was 83(80%).54 benign lesions were diagnosed by conventional ultrasound,while the actual number was 73(73%).The sensitivity of MG in the diagnosis of breast neoplasm was 80.7%,as well as 74.0% for specificity,and 77.6% for accuracy.The difference between the conventional ultrasound and MG in terms of sensitivity,specificity and accuracy had no statistical significance(P>0.05).The sensitivity,specificity and accuracy of ABVS were both higher than those of conventional ultrasound and the difference was significant(X2=11.65、4.30、15.39,P< 0.05).The degrees of sensitivity and accuracy of ABVS were both higher than those of MG,whose differences had statistical significance(X2=10.53 、 13.49,P < 0.05).The difference of specificity of them had no significance(X2=3.58,P=0.06).(4)The area under the ROC curve of conventional ultrasound,ABVS and MG were respectively0.759,0.936 and 0.763.(5)The sensitivity of convergence phenomenon that diagnosed the tumors of breast was 44.9%,as well as 100% for specificity and 68.3% for accuracy.(6)As for ABVS and MG,the diagnosis of breast calcification was comparatively consistent(Kappa=0.79,P< 0.05).Conclusions:The three-dimensional imaging,full-volume scanning and specific coronal plane image of ABVS can clearly show the breast neoplasm,which can definitely improve the accuracy of diagnosis.As for ABVS and MG,the diagnosis of breast calcification was comparatively consistent and ABVS can make up the conventional ultrasound’s deficiency in calcification,which improves the diagnosis of both malignant and benign tumors.Compared to the conventional ultrasound and MG,ABVS has better performance in sensitivity,specificity and accuracy,which is of great significance in clinical diagnose. |
