Objective: To observe the orthodontic design and clinical characteristics of a case of Angle Class Ⅲ malocclusion with impacted teeth which treated by extraction of four first premolar.Materials and Methods: A case of Angle Class Ⅲ malocclusion with impacted teeth,treated by extraction of four first premolars,was selected from orthodontic department of Fujian Stomatology Hospital during postgraduate stage.The patient was a 12-year-old girl with her chief compliant that “irregular teeth and missing canine in the upper arch”.It is symmetrical in front.Her profile was convex with Class Ⅲ molar relationship.Permanent dentition could be seen except second molars and left canine in upper arch.The lower second molar in the right side was mesially impacted.Overbite and overjet were normal while the lower midline deviated to right about 1mm.Upper midline was coordinated with facial midline.Her dental crowding was 9mm in the upper and 9.5mm in the lower.She had a lot of decayed teeth and the lower incisors were lingual inclination.Panoramic Radiography showed the left canine in upper arch was impacted in the alveolar bone.The analysis of the cephalometry was performed a ClassⅠskeletal pattern with an high angle.Upper incisors were proclined and lower incisors were straight in a constricted mandible.The case was treated by extraction of four first premolars with straight wire appliances.At last,(?)were carried into arch by guiding-eruption operation and auxiliary arch after leveling and aligning.closing space to establish ideal occlusion with normal overbite and overjet.Result: At the end of the orthodontic treatment,the ideal overjet and overbite is achieved.(?)were carried into arch.Extracted space almost closed with a good alignment and nice occlusion.The profile is improved compared to that of pretreatment.Conclusion: Satisfactory curative effect of removing first premolars in the treatment of impacted teeth could be achieved through the right design and operation.
