Objectives: This study is aimed to the explore the levels of bone turnover markers in patients with osteoporosis in order to understand the situation,assess the risk of fractures of bone turnover and guiding anti-osteoporosis treatment.Methods: Choose 615 osteoporosis patients who live in May 2014 to May 2016 as the research object with 474 females(77.1%)and 141 males(22.9%),whose ages are all more than 50 and average age is 64.92±9.02.Collect patients’ clinical information;Detect blood blood calcium,phosphorus,alkaline phosphatase(ALP),25-hydroxyl-vitamin D,parathyroid hormone(PTH),collagen type 1 carboxyl end peptide beta special sequence(beta CTX),osteocalcin(MID),total collagen type 1amino terminal extension of the peptide(P1NP),fasting blood glucose,blood uric acid(UA)and blood lipids,application of dual-energy X-ray absorptiometry(dexa)detection of bone mineral density;Statistical analyses were performed using SPSS22.0 for Windows,and P < 0.05 for the difference is statistically significant.Results:1.After adjustment for age and diabetes,female patients’ P1 NP,β-CTX and MID level were higher than male ones(all P<0.05).Female patients’ β-CTX,P1 NP and MID level were getting lower at the age of 50 to 59,60-69 and 70-year-old age group,but the differences were not significant(all P<0.05).2.Females’ measurement of bone mineral density of lumbar spine was signifi cantly lower than males’(P<0.05).And female patients’ femoral neck BMD val ue had no significant difference compared with the male(P > 0.05).3.After adjustment for age,sex and BMI,osteoporosis group of patients with diabetes mellitus β-CTX,P1 NP and MID level were significantly lower than non-diabetes.4.In 615 cases of osteoporosis,202 people(32.8%)had osteoporosis fracture.After correcting age,sex and BMI,there was no significant difference between fracture group and non fracture group in β-CTX,P1 NP and MID level(P>0.05),and the BMD of lumbar vertebrae and femoral neck was significantly lower in fracture group than in non fracture group(P<0.05).5.Female osteoporosis fracture did not receive drug treatment for 105 people(58%),foundation treatment(supplement calcium and vitamin D)for 45 people(24.9%),foundation treatment with bone resorption inhibitor therapy for 31 people(17.1%);Male osteoporosis fracture took foundation treatment for 9 people,and the left did not receive treatment.6.Patients 470 cases(76.4%)had 25-hydroxy-vitamin D deficiency,25-hy droxy-vitamin D insufficient for 111 cases(18%),25-hydroxy-vitamin D norm al for 34 cases(5.5%).Female patients’ 25-hydroxy vitamin D was lower than male,no significant differences(P>0.05);The level of 25-hydroxy-vitamin D in women with osteoporosis was significantly lower than that in Non-diabetic gro up(P<0.05);There was no significant difference between 25-hydroxy-vitamin D level and non fracture group in patients with osteoporotic fracture.Conclusions:1.Female bone resorption and bone formation were active than men in patient s with osteoporosis and females were more suitable for the application of bone res orption inhibitor;2.Vitamin D deficiency accounted for 76%,suggesting that vitamin D sup plementation could not be neglected in the treatment of osteoporosis;3.In diabetic patients,bone mineral density and vitamin D levels were lo wer than those of non-diabetic patients,suggesting that the prevention and treat ment of osteoporosis should be emphasized.4.More than half of patients with osteoporosis after fracture were not rec eiving medication,Tip osteoporosis education should be strengthened in order t o raise awareness of osteoporosis and reduce the incidence of fractures.
