| According to the NIH consensus,a new classification of prostatitis is defined including chronic prostatitis / chronic pelvic pain syndrome(CP/CPPS)as Type III prostatitis.Chronic prostatitis/chronic pelvic pain syndrome(CP/CPPS)is a complex clinical entity composed of genito-urinary pain,lower urinary tract symptoms,and/or sexual dysfunction lasting for at least 3 of the last 6 months.The etiology and pathogenesis are complicated and unclear and no efficient monotherapeutic option is available.Thus CP/CPPS has become problems in clinical area.Due to the growing acceptance in the use of acupuncture in urology as a result of disappointing evaluation of standard therapy for CP/CPPS,there is a need to Standardized acupuncture treatment program as there is a lack of coherent treatment strategy and operation technique.Objective:Review the pathogenesis of CP / CPPS that has been reported.Summarize the rules of acupuncture strategy in the reported treatment of CP/CPPS and give a look into the recent advent of clinical research combined with its possible therapeutic mechanism,in order to provide a standardized acupuncture method to this condition.Method: A CNKI,Wanfang data resource,VIP databases and PubMed search of all recent relevant articles are searched using the keywords/phrases : CP,CPPS,chronic prostatitis,chronic pelvic pain syndrome,Chronic prostate pain.1.Combined with Pathogeny,Pathogenesis or Research progress both in english and Chinese to summary the pathogenesis and make a conclusion.2.Combined with acupuncture,electroacupuncture,needle or electro-needle both in english and Chinese to calculate the frequency of acupoints and meridians respectively in terms of acupuncture therapy alone and acupuncture therapy combined with other treatment strategies.The number of acupoints used,frequency of operation and treatment course in a single literature has also been recorded and taken into consideration.3.Make a search combination of acupuncture,electroacupuncture,needle,electro-needle with mechanism or function both in english and Chinese,and then s earch within results according to the key words ahead to summarize the mechanis m of acupuncture in the treatment of CP/CPPS.Result:1.This study summarized domestic and foreign reviews about the etiology and pathogenesis of CP/CPPS and realized that there still lies controversial issues as a result of unclear understood with this process.There are theories such as infection mechanism theory,urine reflux mechanism theory,doctrine of autoimmune mechanism,the neural mechanism to regulate and control theory,theory of the social psychological factors,as well as hormone levels and genetic inheritance in triggering the pathogenesis process.Treatment programs should be taken according to the various pathogenesis and clinical manifestation.2.A comprehensive review of published randomized controlled trials(RCTs)on the treatment of CP/CPPS and practical best evidence recommendations for management We finally enrolled 216 original research articles which considered psychosocial variables.3.Considering the mechanism of action in the treatment of CP / CPPS,the reaserch of acupuncture mainly focus on the neural regulation,immune regulation and analgesic effect,in which the last mechanism has been far more concerned about.These effects of acupuncture could be explained by the following hypothetical mechanisms: local peripheral events in the tissue,spinal(segment)mechanisms,and supraspinal(extrasegmental or central)mechanisms.On the other hand,previous statistical results in the second part of this research Highlights the traditional Chinese medicine theory of “where the meridian extends to,where lies the treatment effect on”and take measures of the local therapeutic effects remote therapeutic effect and Special therapeutic effect of A certain point.Conclusion:Acupuncture is widely used in the treatment of CP/CPPS both in clinical and reaserch activities.When the pathogenesis is unknown,and a single treatment effect failed to meet the promising results,the acupuncture treatment bloomed into an important method because of the Excellent clinical efficacy where underlyies the unique theory of channels and collaterals theory,as well as the modern theory of neural regulation,immune regulation and analgesic effects?Further study need to take concern to to establish a standardized acupuncture treatment program so as to promote the standardization for the treatment and research of CP/CPPS based on the results of the study?... |
