Objective To evaluate the effects of pelvic floor function after Laparoscopic total hysterectomy(LTH)?Transabdominal hysterectomy(TAH)and the effect of Kegel exercise on pelvic floor function recovery after hysterectomy.Methods Collect in January 2015-September 2016,for uterine benign diseases do hysterectomy,a total of 160 people with non-prolapsed uterus.The people were divided into two groups receiving TAH and LTH surgery,each group has 80 people before operation.Then the LTH is divided into the treatment group and the non-treatment group,each group has 40 cases(group a,b),the TAH also randomly be divided into treatment group and non-treatment,each group has 40 cases(group c,d).The treatment group(group a?c)do the Kegel exercise,non-treatment group(group b?d)do not the Kegel exercise.After 1month?3 months?6 months?1 year follow-up,each time evaluate the postoperative pelvic floor function in patients with pelvic floor electrical stimulation,gynecological examination,clinical evaluation.Finally,collecting data and sorting data,comparing the effects of pelvic floor function after two kinds of hysterectomy and the effect of Kegel exercise on pelvic floor function recovery after hysterectomy.Results 1.Before operation,the TAH and LTH each group has 80 people.The patient's general condition is compared between two groups,the results showed without statistically significant difference(P > 0.05),comparable.Postoperative compare LTH(group b)and TAH(group d),two groups of postoperative 1 month not seen obvious abnormity,and postoperative 3 months,two groups only appeared stress urinary incontinence,but no obviousdifference was found between the number of cases,so no statistical difference.Group b patients postoperative 6 months and 1 year in the degree of vaginal prolapse 1case and 3cases,6 months and 1 year postoperatively in patients with group d 3 cases,5 cases appeared,the contrast between the two groups of the indicators,without significant difference,P > 0.05.Group b patients postoperative 6 months and 1 year in stress urinary incontinence each 3cases and 6 cases,6 months and 1 year postoperatively in patients with group d appeared in11 cases and 14 cases,contrast between the two groups of indicators,with significant differences,P < 0.05.1 months postoperatively in patients with group b and group d compared?,? muscle fiber electromyography values,without statistical significance(P >0.05),postoperative 3 months,6 months and 1 year comparing differences between the two groups was statistically significant(P < 0.05).2.Transabdominal hysterectomy in the treatment group c and non-treatment group d,postoperative 1 month not seen obvious abnormity,two groups of postoperative 3 month only appeared stress urinary incontinence(2cases of group c and 3 cases of group d),but still no obvious difference between the number of cases,so without statistical significance.After postoperative 6 months and 1 year patients of group c in the vaginal prolapse 2 cases and 3 cases.Group d patients each has 3 cases and5 cases,with comparison between the two groups,without significant difference,P > 0.05.After postoperative 6 months and 1 year patients of group c appear stress urinary incontinence in 5 cases and 9 cases,group d patients each has 11 cases and 14 cases,with comparison between the two groups,without significant difference,P > 0.05.After postoperative 1 month in patients with group c and group d compared ?,? muscle fiber electromyography values,without statistical significance(P > 0.05),postoperative 3 months,6 months and 1 year comparing differences between the two groups,with statistically significant(P < 0.05).Conclusion The pelvic floor function is affected in two hysterectomy;The pelvic floor function were more impact after Transabdominal hysterectomy than Laparoscopic totalhysterectomy,easily lead to the occurrence of stress urinary incontinence.Insist for a long time to Kegel exercise on the recovery of early postoperative pelvic floor muscle strength has great effect.
