| Objective:Cochrane system evaluation method for hemiarthroplasty differences in the clinical efficacy of bone cement and cementless prosthesis systematic evaluation.To provide evidence for clinical in selection of appropriate prosthesis.Methods:We retrieval the major Chinese and English database by computer,such as MEDLINE,Pub Med,Ovid and Wanfang,VIP,CNKI,retrieve time from database time to May 2015,Manual search related journals,and use such as Google online resources enlarge the retrieval area,according to the inclusion and exclusion criteria,retrieves randomized controlled trials about cemented and cementless hemiarthroplasty for treatment of femoral neck fractures in the elderly,Read the full text by two persons respective,extract data and summarization,using Rev Man5.3 software to Meta-analysis then.Results:A total of 18 RCTs were included,2308 cases hips,Left 1178 cases,right 1130 cases,the results of meta-analyses showed that b one cement longer operationtime [MD =8.30,95% CI(6.86,9.74),P<0.001],more bleed loss [MD =38.72,95% CI(0.11,77.32),P =0.05)].few prosthesis complications [RR =0.29,95% CI(0.18,0.45),P <0.00001],few one year postoperatively thigh pain [OR =0.69,95% CI(0.55,0.87),P =0.002],There was no significant difference in revisio n rate[OR=0.91,95%CI(0.59,1.41),P=0.68],cardiovascular complic ations[OR=1.03,95%CI(0.63,1.70),P=0.91],Mortality[OR=0.89,95%CI(0.73,1.07),P=0.21]and the rate of deep venous thrombosis[OR=1.10,95%CI(0.43,2.86),P=0.84].Conclusion:Compared with the cementless group,bone cement s urgery need longer time,more bleed loss,however,there were advantag es in complications and pain,and did not increased the mortality rate,revision rate and deep vein thrombosis. |
