Objective:To study the carotid plaques and blood homocysteine levels both joint for coronary heart disease have predictive value and carotid plaques Crouse integral,blood levels of homocysteine and the correlation between the severity of coronary artery lesions.Methods:To collect the first hospital of shanxi medical university of cardiology in July 2015 to August 2016 hospitalized patients with coronary heart disease examination,conforms to the inclusion and exclusion standard in 205.All entrants are general information collection,parallel assay blood homocysteine,neck vessels color Doppler ultrasound,coronary angiography examination,according to the results of coronary angiography,divided into two groups,the case group 108 examples,and the control group 97 examples.Carotid plaques,the high homocysteine levels(HHcy)and carotid artery plaque joint HHcy are calculated respectively the sensitivity of diagnosis of coronary heart disease(CHD),and compares the evaluation index of different diagnostic test.Judgment of carotid plaques Crouse integral,blood homocysteine level and the correlation between the severity of coronary artery lesions.Results:1.The comparison between different risk factors for coronary heart disease,such as high blood pressure,diabetes,dyslipidemia,family history and so on,there is no difference between two groups(P > 0.05);Between the two groups of carotid plaques and blood Hcy levels differ(P < 0.05).2.The Se of carotid plaques in the diagnosis of coronary heart disease is 82.4%,the Sp is 35.0%,the PPV is 58.6%,the NPV is 64.2%.3.The Se of HHcy in the diagnosis of coronary heart disease is 63.0%,the Sp is 38.1%,the PPV is 53.1%,the NPV is 48.0%.4.The Se of carotid plaques with HHcy tandem combined diagnosis of coronary heart disease is 52.8%,the Sp is 59.8%,the PPV is 59.4%,the NPV is 53.2%;The Se of Carotid plaques with HHcy parallel diagnosis of coronary heart disease is 92.6%,the Sp is 12.4%,the PPV is 54.1%,the NPV is 60.0%.5.Pearson correlation test shows that: in 89 cases of coronary heart disease(CHD)patients with carotid artery plaque positive,Gensini score with Crouse integral correlation coefficient is 0.234(P = 0.027);Gensini score and blood Hcy levels of correlation coefficient is 0.250(P = 0.018).Conclusion:1.carotid artery plaque and blood homocysteine levels both combined forecast for coronary heart disease have a certain value,the combined prediction value is superior to the single factor of the two to the predictive value of coronary heart disease.2.Carotid plaques Crouse integral,blood homocysteine level have correlation between severity of coronary artery lesions.
