Clinical Analysis Of Conservative Treatment And Surgical Treatment Of Distal C-type Fractures In The Elderly

Objective:To analyze the curative effect of conservative treatment and surgical treatment of distal C-type fractures of the elderly in the elderly,and to evaluate the clinical curative effect of this treatment.Method:Collected in Shanxi Medical University Second Hospital from January 2014 to June 2016 were diagnosed as distal radius fractures in patients with,according to the inclusion and exclusion criteria to determine the experimental study of a total of 42 cases.According to the treatment methods used in patients were divided into two groups: the surgical group of open reduction plate fixation group(open reduction plate fixation)and conservative group(closed reduction plaster external fixation).There were 18 males and 7 females,aged 60-77 years,mean age(67.13 ± 7.136)years;fracture type,C1 type 9 cases,C2 type 18 cases,C3 type 4 cases.There were 17 patients in the conservative group,9 males and 8 females,mean age(67.55 ±6.806)years old;fracture type,C1 type 7 cases,C2 type 8 cases,C3 type 2 cases.Retention of preoperative,postoperative and 6 months of imaging data.The healing conditions of incision and infection,palm tilt angle,ulnar angle and radius recovery were observed.The healing time of fracture and the excellent wrist function of Cooney frequency were observed.Statistical analysis of the observed data to compare the advantages and disadvantages of the two treatment programs.Result:Two groups of patients were well followed up,the wound healing in the surgical group was good,no necrosis of the skin,no infection occurred.(10.86 ± 1.582)weeks,the conservative group was(10.43 ± 1.304)weeks,the two groups of patients with fracture healing time,the difference between the two groups of patients with fracture healing time,no difference No statistically significant(P> 0.05).The recovery of the postoperative palm tilt angle,ulnar angleand radius of the two groups were better than those of the conservative group,the difference was statistically significant(P <0.05).There was no significant difference in the excellent and good rate of wrist function between the two groups(P> 0.05).There were two cases of postoperative complications,one case of flexor tendon injury,one case of traumatic arthritis.Conservative group appeared two cases of complications,one case of shoulder hand syndrome,a case of traumatic arthritis.There was no significant difference in the incidence of complication between the two groups.Conclusion:Although most of the efficacy analysis at home and abroad confirmed that the internal fixation system for distal radius of the C-type fracture of the treatment,the nail plate system can provide a more satisfactory reset and more fixed,coupled with early functional exercise,improve patient quality of life,surgery Treatment has become the mainstream choice for such fractures.But it is undeniable that the surgical reduction of conservative treatment is still the elderly radius of distal C-type fracture treatment of the feasibility of choice,especially in the face of serious crushing and poor soft tissue conditions.Therefore,clinicians in the development of such fractures of the treatment program,to fully consider the patient’s wishes,economic conditions and fracture types and other factors,the specific circumstances of the specific analysis.
