The Effect Of Community Intervention On Type 2 Diabetes Patients

[Objective] The objective of the study is to evaluate the effectiveness of the type 2 diabetes community intervention in city and find the influencing factors to the effectiveness.for type 2 diabetes community intervention methods provide a scientific basis for the selection and optimization.[Method] 1.Using cross-section study,collect type 2 diabetes and other health related information in a community in City during March to June,2015.Using single factor chi-square test and multiple Logistic aggressions,analyze the epidemiological characteristic,self-intervention condition,community intervention condition and influential factors.2.Type 2 diabetes individual intervention methods explore: Classify the participants into strengthened health comprehensive intervention group,psychological intervention group and control group.Collect the participants' figures on blood glucose and glycosylated hemoglobin before and after three months' intervention and follow up.Through questionnaire and SDS scale to obtain the knowledge,behavior and compliance of the participants.Test the intervention effect difference between the three groups by t-test?chi-square test and variance analysis.3.Type 2 diabetes community intervention effectiveness study: Four community health service centers in a city were randomly divided into two groups: intervention group and control group.Collect FBG(fasting blood-glucose)before and after 6 months' intervention.Assess the knowledge,behavior and compliance of the participants by questionnaire.Compare the difference between the two groups by Independent sample t-test.[Results] 1.Totally enrolled 1089 type 2 diabetes patients,Investigation rate is 96.2%.collect 1034 effective questionnaires,the effective rate is 94.9%.The prevalence rate of type 2 diabetes in community is 5.69%(95%CI: 5.35%-6.03%),the standardized prevalence rate is 5.03%(95%CI: 4.46%-5.55%).The blood glucose control rate for type 2 diabetes patients in the community was 63.8%(95% CI: 65.50-59.50%).The results of multiple logistic regression analysis show that the protective factors for blood glucose control are taking medicine on time(OR=0.729,95%CI: 0.551-0.965),family supervision(OR=0.694,95% CI: 0.517-0.932)and diet control(OR=0.177,95% CI: 0.087-0.362).The negative factors for blood glucose control are lack of blood glucose monitoring(OR=1.359,95%CI: 1.008-1.831),no exercise(OR=1.536,95%CI: 1.136-2.078)and overweight or obesity(OR=1.123,95%CI: 1.031-1.223).2.The experimental epidemiology study showed that the FBG,glycosylated hemoglobin,awareness rate and the grade of SDS scale improved significantly(P<0.05)in comprehensive intervention group and physiological intervention group.And the comprehensive intervention group achieved the best effect in FBG,glycosylated hemoglobin,awareness rate,behavior improvement rate than control group(P<0.05),the physiological intervention group achieved the best effect in SDS sale(P<0.05).3.Community experiment shows that the FBG has improved in both Intervention group and control group.And the Intervention group got better mean value of knowledge,behavior and compliance than control group(P<0.05).[Conclusion] 1.The poor blood glucose control of type 2 diabetes is still one of the major problems in the prevention and control of chronic disease in community-based communities.The protective factors for blood glucose control are taking medicine on time,family supervision and diet control.The negative factors for blood glucose control are lack of blood glucose monitoring,no exercise and overweight or obesity.2.The strengthened comprehensive intervention and physiological intervention,are effective for the control of blood glucose and glycosylated hemoglobin in individual and could improve the awareness rate and behavior,and the effectiveness is better than other intervention methods.3.On the basis of the state intervention,it is important to improve the intervention mode and content of community type 2 diabetes mellitus,which is effective to improve the intervention effect of type 2 diabetes patients.
