Objectives:The paper discusses common risk factors and etiology type for cerebral infarction in youth,compares with different common etiology type and common risk factors of different genders for cerebral infarction in youth,and provides basis for clinical diagnosis,treatment and prevention.Methods:55 patients with acute cerebral infarction in youth between 18 and 44 years old who were hospitalized in neurology of the Second Hospital Affiliated to Soochow University from January 2015 to December 2016 were selected to collect relevant clinical data(including hospitalized medical history of patients and imaging manifestations,etc).Related risk factors were selected to do retrospective analysis of risk factors and etiology type by combining with TOAST etiological types,so as to discuss etiology type and risk factors of gender differences for cerebral infarction in youth.Results:55 patients with cerebral infarction in youth included 35 patients in 2015 and 20patients in 2016 and contained 39 male patients(accounting for 70.9%)and 16 female patients(accounting for 29.1%).The male and female proportion was about 2.44:1.20patients ranged from 18 years old to 35 years old,occupying 36.4%of total patients with male and female proportion of 3:1.35 patients ranged from 36 years old to 44 years old,occupying 63.6%of total patients with male and female proportion of 2.18:1.Gender proportion difference of patients at different age groups was conducted statistical analysis(X~2=0.225,P=0.614),P>0.05,and the difference had no statistical significance.55 patients included 15 patients with atherosclerosis(27.3%),7 patients with cardioembolism(12.7%),11 patients with vascular occlusion(20.0%),9 patients with other etiology(16.4%),and 13 patients with unknown etiology(23.6%).In these five etiological types,atherosclerosis had the largest proportion,followed by unknown etiology,while vascular occlusion(20.0%)and other etiology(16.4%)also had considerable proportions.There was no statistical significance in etiology of different genders.In risk factors of cerebral infarction in youth,it generally included the following factors according to frequency of occurrence,including 37 patients with dyslipidemia(67.3%),28 patients with history of alcohol intake(50.9%),27 patients with smoking history(49.1%),23 patients with hypertension(41.8%),22 patients with triglyceride(40.0%),20 patients with elevated LDL-C(36.4%),16 patients with family history(29.1%),7 patients with heart disease(12.7%),7 patients with diabetes(12.7%),5patients with high cholesterol(9.1%),and 4 patients with hybrid hyperlipemia(7.3%).In addition,male patients with hipertrigliceridemia(19 male patients and 3 female patients,P=0.039),smoking history(26 male patients and 1 female patient,P<0.01)and drinking history(26 male patients and 2 female patients,P<0.01)were obviously higher than female patients,showing that their difference had the statistical significance.As for hypertension,diabetes mellitus,dyslipidemia,family history and heart disease,there was no statistical significance between male patients and female patients(P=0.309,P=1.000,P=0.080,P=0.450,P=1.000 successively).The difference between dyslipidemia and hyper-triglyceride had the statistical significance(P<0.05).There was no statistical significance in elevated LDL-C,hypercholesteremia and hybrid hyperlipemia.Conclusions:1.Gender proportion of patients with cerebral infarction in youth at different age groups had no obvious difference.2.Patients with cerebral infarction in youth with different genders had diverse risk factors,but no evidence showed that there were different etiological types between them.3.In cerebral infarction in youth,males had the larger proportion.Male patients were more exposed to hipertrigliceridemia,smoking and drinking.LAA and UE in common etiology had the largest proportion,while SAO and OE also had the considerable proportion.For patients with cerebral infarction in youth,especially for patients with unknown etiology,it is necessary to improve comprehensive evaluation of blood vessel and ultrasonic cardiogram and define etiology at early stage.Meanwhile,patients should complete secondary prevention of stroke and intervene in risk factors.
