Objective:The work load of medical staff are heavy and unstable.Medical personnel not only face the risk of infection by various bacteria and viruses,but also deal with more and more tense doctor-patient relationship.Medical staff has become a high risk group of occupational stress and job burnout.At present,there are few studies on the relationship between occupational stress and job burnout,and the research is focused on the medical staff of the third grade hospital.There is less research on the psychological status of the medical staff for two level hospitals.This study used cluster sampling method to select 2 two level of hospital medical personnel as the research objects.Occupational stress factors,occupational stress reaction,personality characteristics,relaxation factors and burnout Inventory were investigated to analyze the characteristics of occupational stress and burnout,evaluated the influence of occupational stress on Job burnout to find the influencing factors of occupation burnout,and puts forward the specific intervention measures according to different influencing factors,provide the basis for screening,diagnosis,job burnout crowd prevention and intervention.Methods:A total of 680 medical personnel from two second-class hospitals in Yangzhong City and Chifeng City were selected as the research objects,using cluster sampling method,and 656 valid samples were obtained.Occupational stress factors,occupational stress response,personality traits,mitigation factors and occupational depletion were tested using the Occupational Stress Index Scale and the Maslach Occupational Exhaustion Questionnaire.Using Epidata input data,Excel2013 and SPSS 19.0 software package to build the database and analyze it.Using mean and standard deviation to describe the quantitative data,the number of cases and the percentage are used to describe qualitative data.Statistical analysis and comparative analysis were carried out by multivariate analysis of variance,single factor analysis of variance and chi-square test.Simple correlation analysis,principal component stratification regression were used to analyze the influencing factors of occupational stress,job exhaustion and its three dimensions.And using the principal component discriminant analysis to establish the discriminant equation of occupational exhaustion and verify the equation.Results:1.The survey of occupational burnout positive number of 2 hospitals are 406 people,accounting for 61.90%;negative number has a population of 250,accounting for 38.10%.The positive rate of occupational burnout among medical workers in a two grade hospital in Yangzhong is 53.5%,and the positive rate of occupational burnout in a hospital of Chifeng is about 74.6%.2.There were significant differences in occupational stress and job burnout among different demographic characteristics:? The multivariate variance analysis showed that there were significant differences in the scores of occupational stress factors,occupational stress response,personality traits,mitigation factors among different regions,gender,age,educational level,personal monthly income(P<0.05);there was a significant difference in job burnout among different areas,education level,marital status and monthly income groups(P<0.05);?Chi square test showed that there were significant differences in the positive rate of job burnout among different areas,educational level,marital status and monthly income(P<0.05);the positive rate of job burnout in Western China was higher than that in Eastern China(P<0.05);The positive rate of occupational burnout of married population was higher than that of unmarried population(P<0.05);high school(technical secondary school)and the following groups,occupation burnout rate was higher than the group of college and above(P<0.05).?The job burnout positive rate of monthly income of<2000 group was higher than the income 2000?3000 group(?2?12.357,P<0.001)and>3000(?2=11.325,P=0.001);?There was no significant differences in the positive rate of job burnout among different genders.3.The effects of occupational stress,personality and alleviating factors on occupational stress reactiones:?Occupational stress factors and occupational stress correlation analysis results showed that the stronger the control ability and the more opportunities to enhance participation in decision-making,the more satisfied with the work;the more serious work conflict and the more use of technology fully and the more monotonous work risk and the greater the responsibility,the more not satisfactory with the work and the more severe depression and the more body complain and daily more nervous;the more load faster and the heavier quantitative load,the more complaining the body is and the more depressive symptoms and the more serious daily life stress is;the stronger the control task,the more serious symptoms of depression.?The personality characteristics and occupational stress correlation analysis showed that self-esteem is stronger and more loyalty to the organization,the more satisfied with the work and the more depressive symptoms and the more daily stress;The more serious of type A behavior and the locus of control on source tendency of external control and the lower organizational loyalty is,the less satisfaction with the work,the more serious of depression and the more serious of physical complaints and the more serious of daily stress.?Mitigating factors and Occupational stress reaction correlation analysis results showed that the more social support,the more satisfied with the work and depressive symptoms and somatic complain less and daily tension lighter;The task strategy and logic to work better and less investment and better time management skills,the fewer depressive symptoms and less body complain and the lower daily stress.The results of principal component regression showed that factors positively predicted job satisfaction scores are job control,coping style;factors negatively predict job satisfaction are work conflict,job demands,job monotonous;the positive predictor of depression symptom score variables are work conflict and job demands,job monotonousness,locus of control,social work support;negative predictors of depressive symptoms were variables of ambition and coping style;the variables of positive prediction of physical complaints are job conflict,job demands,job monotony,and social support.The variables of negative prediction of physical complaints are coping styles4.The effect of occupational stress on job burnout:?Occupational stress factors and occupational burnout correlation analysis results showed that the more monotonous and more conflict and more danger and more job responsibilities and more heavy work load and the faster the rate of change and the more fuzzy of role conflict,the more emotional exhaustion and the more serious of depersonalization;the stronger of task control ability,the more serious depersonalization is;the more worse of work ability to control,the more serious emotional exhaustion;the more serious of group conflict and the faster the load changes and more monotonous dangerous role increasingly blurred and the lower job control ability,the more serious personal accomplishment reduced;?The correlation analysis between occupational stress and job burnout showed that the lower job satisfaction and the more depressive symptoms and physical complaints and the stronger the daily stress,the more serious occupational burnout;?The personality and job burnout correlation analysis showed that mental control of work tend to be external and loyalty is lower and lower self-esteem,emotional exhaustion and depersonalization are more serious;The more aggressive,more serious depersonalization;the more tend to the type A personality,organizational loyalty is lower,lower self-esteem,the more serious personal accomplishment decreased;?Mitigating factors and job burnout correlation analysis showed that the less social support,the higher the degree of job burnout;with good strategy and logic,good job involvement and good relationship between family and work,the degree of job burnout is low.Principal component regression analysis showed that:the variables that positively predicted emotional exhaustion were job conflict,job demand,and social support.The negative variables were the job satisfaction and coping style;the variables that positively predicted the depersonalization score were job conflict,job demand and job monotony;Positive predictor of reduced personal accomplishment scores were work conflict,job demands,job monotonousness,job risk;negative predictors of reduced personal accomplishment scores were participation in decision-making,job control,job satisfaction,coping style;the positive predictors of job burnout scores were job conflict,job demands,job monotony,and negative predictors of job burnout.Conclusion:Occupational stress and job burnout of medical staff are affected by demographic characteristics.The occupational stress are affected by area,gender,age,marital status,personal monthly income;occupational burnout status are affected by area,education level,marital status,personal monthly income;occupational stress factors,personality characteristics and mitigating factors and occupational stress reactions are closely related;part variables of occupational stress factors,personality characteristics and mitigation factors could predict occupational stress reactions;Occupational stress is closely related to job burnout;Some occupational stress variables can lead to burnout.
