| ObjectiveSince the outbreak of the novel coronavirus in late 2019,medical workers have been faced with heavy work tasks,regular overtime work,and constant changes in epidemic prevention and control.Based on the long term epidemic prevention and control situation,this study,through research and investigation,understands the current situation and influencing factors of occupational stress and occupational burnout of primary medical staff in western Yunnan,and analyzes the relationship between occupational burnout and occupational stress in various dimensions.Combined with the interview,the source of occupational pressure and the understanding of job burnout of grassroots medical workers were deeply explored.In order to alleviate the job burnout of primary medical staff and guarantee their mental health,relevant policies and suggestions are put forward,so as to provide reference for the stability of the talent team at the primary level and the improvement of the health service ability of primary medical institutions.MethodsIn this study,stratified and multi-stage sampling method was adopted to investigate 16 primary medical units in West Yunnan,and a total of 786 primary medical personnel in West Yunnan were finally included in the study.The questionnaire included general demographic characteristics,health behavior patterns,the Maslach Burnout Inventory General Edition and the Clinician Occupational Stressor Inventory.In this study,7 primary medical staff were interviewed in depth using the selfdesigned Structured Interview Outline of Occupational Stress of Primary medical Staff.Epi Data3.1software was used to double input the questionnaire,and IBM SPSS26.0 and R.4.2.0 software(ggplot2,qgraph and epicalc software packages)were used for data processing and mapping.Quantitative variables were described statistically by means and standard deviation,while qualitative variables were described statistically by frequency and percentage.In univariate analysis,t test and variance analysis were used,and multiple linear regression was used to explore the influencing factors of occupational stress and job burnout.ResultsA total of 802 questionnaires were collected in this study,and 786 were valid,with a recovery rate of 98.0%.(1)The average score of job burnout of primary medical staff was 34.68,and the detection rate of job burnout was 53.3%.Personal sense of achievement was the most serious,followed by cynicism,and emotional exhaustion was the lowest.(2)The average score of occupational stress was 2.53±0.40,indicating that occupational stress was above the medium level.Organizational environment,work load and doctor-patient relationship were the main sources of stress for primary medical staff.(3)Age,monthly income,weekly exercise time,exercise intensity,weekly working time and self-rated health showed statistically significant differences in job burnout scores.(4)Age,working years,education background,marriage,monthly income,employment form,job title,position,night shift,sleep time,weekly working time and self-rated health had statistically significant differences in occupational stress scores.(5)There is a correlation between occupational stress and job burnout in all dimensions.(6)There are various sources of stress for primary medical staff,and the most frequent words are work pressure,economic pressure and life pressure.(7)At present,there are some problems in primary medical activities,such as lack of hardware equipment,extra tasks and unreasonable organizational structure.(8)Grassroots medical staff have insufficient understanding of job burnout and are one-sided.ConclusionThis study found that the job burnout of primary medical staff was obvious,and the occupational stress level was above the medium level.Grassroots medical workers face problems such as overtime work,lack of sleep and low income.There is an obvious correlation between occupational stress and job burnout. |
