Objectives To investigate the occupational stress, job burnout and anxiety levels of medical staff in a Grade A general hospital in Hebei Province; To analyze the influence of individual characteristics on occupational stress, job burnout and anxiety of medical staff; The relationship among occupational stress, job burnout and anxiety was analyzed and discussed, and the mediating effect of job burnout on occupational stress and anxiety was discussed,so as to provide a basis for reducing occupational stress and improving mental health level of medical staff. Methods From November 2020 to January 2021,a cross-sectional survey was conducted to investigate the basic situation, occupational stress, job burnout and anxiety of 1346medical staff in a third-grade grade A general hospital in Hebei Province.Desptive analysis, rank sum test,χ~2 test,Spearman rank correlation analysis, and multivariate Logistic regression analysis were performed for the data.In the descriptive analysis, the measurement data that satisfied the normal distribution was represented as?X±S,while those that did not were represented as the median(M)and the interval between quinterles(Q).A structural equation model based on nonparametric percentile Bootstrap method was used to explore the relationship among occupational stress, job burnout and anxiety, and to analyze the mediating effect of job burnout.Inspectionlevelα=0.05.Results 1 Basic information:A total of 1346 questionnaires were sent out and 1150 were received with a recovery rate of 85.44%.Among them,1044 were effective and the recovery rate was 90.78%.Among the 1044 respondents,365(34.96%)were doctors and679(65.04%)were nurses. There were 817 females(78.26%)and 227 males(21.74%).The mean age was(34.04±7.65)years old.The majority of ethnic groups were Han(n=1019,97.61%).The registered residence in the city accounted for a relatively high proportion(n=785,75.19%);The education level was dominated by bachelor’s degree(n=618,59.20%);Married status accounted for a higher proportion(n=783,75.00%);The number of patients in the department of surgery was the largest(n=366,35.06%),followed by internal medicine(n=272,26.05%).The professional titles were mainly distributed in primary level(n=456,43.68%)and intermediate level(n=430,41.19%).2 The occupational stress of medical staff in this hospital was mainly high occupational stress(n=677,64.85%),and the demographic characteristics of 25~29 years old group, intensive medical department and intermediate professional title medical staff suffered higher occupational stress, and the 4001~6000 yuan monthly income group showed a higher incidence rate of high occupational stress.3 The incidence rate of job burnout was 60.63%,of which the incidence rate of light and high job burnout was 52.01%and 8.62%respectively.Demographic characteristics of women,25~29 years old group,non-city registered permanent residence, undergraduate education, intensive care, nurses, intermediate job title,6~10 years of service group, shift work group showed a higher level of burnout, whilefemale,ICU and emergency department medical staff showed higher burnout incidence rate,and>20 years service group showed lower burnout incidence rate.4.The incidence rate of anxiety was 51.53%,including 36.78%of mild anxiety,7.95%of moderate anxiety and 6.80%of severe anxiety.Demographic characteristics of female,non-city registered permanent residence, bachelor’s degree, primary and intermediate professional title, critical care department anxiety level is higher,>20 years of service group anxiety level is lower, while female, critical care medical staff anxiety incidence rate is higher.5 Correlation analysis of occupational stress, job burnout and anxiety:Except that there was no statistical significance in the correlation between reduced personal achievement dimension and occupational stress score and total score of anxiety, the total score of the three scales and all dimensions were correlated to different degrees(P<0.05).Intermediary effect test results show that the occupational stress of the total effects of anxiety, direct effect and indirect effect of point estimates were 0.595,0.238,0.357,reached significant level(p<0.05),and the confidence intervals are not contain 0,indicating a significant direct effect and indirect effect and total effect of job burnout in the intermediary effect between occupational stress and anxiety, intermediary effect value of0.357,accounting for 60%of the total variance. Conclusions Province is relatively prominent, and the degree and incidence of occupational stress are both high. The level and incidence of job burnout are slightly lower than the results of some similar studies, and the level and incidence of anxiety are similar to those of some similar results.Attention should be paid to the labor-reward imbalance, job burnout and anxiety of medical staff with characteristics of 25~39 years old,ICU,non-municipal registered permanent residence, bachelor’s degree, intermediate professional title,6~10 years of working experience, shift system, and middle monthly income level.Job burnout has a partial mediating effect on occupational stress and anxiety, which suggests that the anxiety level of people in this profession can be reduced either by improving occupational stress or by alleviating job burnout.Figure3;Table18;Reference 114...
