"Internet Plus Medical" Under The Background Of Xiamen City Study On The Mode Of Chronic Disease Management

According to relevant data,along with the aggravation of aging population in China,the number of slow patients is also rising.This situation has brought heavy burden to our society and economy.Improving the health of the people is the issue that China's health care reform can never escape.Defect of traditional mode of chronic disease management control separation,optimization of our high-quality medical resources are unevenly distributed.improve health service ability,do prevention health service development,supply and demand balance,to solve the "difficult and expensive" problem is the reform,the only way which must be passed,to explore a new model of chronic disease management.A pilot study on the government in the country and more.Based on the promotion of new medical reform,"this new thing Internet plus medical"rapid development." Internet plus "technology of health technical personnel and computer systems linked to the efficient integration of information resources,realize man-machine cooperation in medical activities." Internet plus "technology can also strengthen the contact between medical institutions,medical personnel can remote guidance using the Internet to receive high levels of doctors,to realize their own service capabilities.Xiamen is one of the chronic disease management pilot city,the implementation of the "family doctor" contracted services within the city,the family doctor team of specialists,general practitioners and health management division,the health technical personnel belonging to different units,for the convenience of the association between the three with mutual aid,strengthen the information interaction between doctors and patients.The Xiamen Municipal Planning Commission will"Internet plus medical" concept used in the "family doctor" contracted services,provide assistance and technical support necessary for the diagnosis and treatment of the family doctor team.This research takes Xiamen which is a chronic disease management pilot base as an example,to explore the Internet technology into the process of chronic disease management in Xiamen City,the specific pattern of structure,operation mechanism,Internet technology evaluation by using the five links in the effect of screening,risk prediction,risk stratification,and comprehensive intervention and early warning Xiamen chronic disease management.And in this case of hypertensive disease,patients with hypertension health data collected from Xiamen Xiangan district hospital,Huli District of Xiamen River Street community hospital two chronic disease management institutions compared,in order to get the promotion of effective mode.In the end,the experience,lessons and extension of the management of chronic disease in Xiamen are summarized.In order to achieve the purpose of this research,we consulted domestic and foreign literature,and summarized the current chronic disease management models put forward by scholars at home and abroad,and analyzed the current research status,in order to provide reference for our chronic disease management mode construction.
