Objective:To compare the clinical curative effect of needle warming moxibustion and acupuncture in the treatment of cold-dampness obstruction syndrome of knee osteoarthritis(KOA),evaluate the clinical value of the two kinds of therapies scientifically and objectively,and provide theoretical and clinical bases for needle warming moxibustion in clinical treatment of KOA,thus better guiding the clinical work.Methods:72 patients with KOA(cold-dampness obstruction syndrome)who were treated in the outpatient clinic and inpatient department of acupuncture of Hubei Provincial Hospital of TCM from January 2016to December 2017 were selected as the subjects.They were divided into observation group and control group by random number table method,35 cases in each group(1 case lost to follow up in observation group,1 case excluded in control group).The control group was given routine acupuncture treatment,and selected acupoints included inside and outside the knee eyes,Heding,Weizhong,Zusanli,Yanglingquan,Yinlingquan,Liangqiu and Xuehai(contralateral),with a sense of the acupoint skin comfort for degree.The observation group was treated with needle warming moxibustion,and selected acupoints included inside the knee eyes(warming needle),outside the knee eyes(warming needle),Heding(warming needle),Weizhong,Zusanli,Yanglingquan,Yinlingquan,Liangqiu,Xuehai,Guanyuan,Shenshu,Quchi(contralateral),with a sense of the acupoint skin comfort for degree.About 2cm of moxa cones were orderly sheathed on the needle handle during the needle retention period,and the needles were removed when moxa cones burned out.The two groups were treated for 30min once,once/d.10d was a course of treatment,and the two groups were given 2courses of treatment,and they were followed up for 2 months.The data were analyzed statistically by SPSS19.0 software.The clinical curative effect,TCM syndrome scores and bone metabolism and related indexes before and after treatment were compared between the two groups.The pain degree[simplified McGill pain scale],lyshlom knee function score scale(LKSS),osteoarthritis index(WOMAC),quality of life(quality of life scale for knee joint disease)and adverse reactions were evaluated.Results:1.In terms of the clinical curative effect in the two groups,the effective rate of treatment in the observation group was significantly higher than that in the control group[91.43%(32/35)vs 71.43%(25/35)](χ~2=4.629,P<0.05).2.In terms of changes in TCM symptom scores in the two groups before and after treatment,scores of heavy knee cold pain,poor flexion and extension,joint stiffness,joint swelling and pale tongue and the total score decreased significantly in the two groups after treatment,and the above-mentioned symptom scores of observation group after treatment were significantly lower than those of the control group(P<0.05).3.In terms of changes in pain degree before and after treatment,McGill scores of the two groups after treatment and after 2 months of follow-up were lower than the same group before treatment,and the scores of observation group were lower than those of the control group at the same time(P<0.05).4.In terms of changes in knee function,inflammatory index and quality of life before and after treatment,the LKSS score of the observation group was higher than that of the control group,while the WOMAC score and quality of life score were lower than those of the control group in 2 months of follow-up(P<0.05).5.In terms of treatment safety,there was no statistical difference in the incidence rate of adverse reactions between the two groups during the treatment[8.57%(3/35)vs 11.43%(4/35)](χ~2=0.159,P>0.05).Conclusion:Compared with simple acupuncture treatment,the curative effect of needle warming moxibustion is better in the treatment of cold-dampness obstruction syndrome of KOA.The latter can significantly relieve symptoms,improve knee function and bone metabolism indexes,decrease the pain,and improve the quality of life of patients,with safety.
