Objective: To investigate the effects of new antiepileptic drugs(oxcarbazepine [ OXC ],levetiracetam [ LEV ],lamotrigine [ LTG ])on semen quality,sexual function,sex hormones and thyroid hormones in male adults with epilepsy.Methods: Thirty-eight newly diagnosed male adult patients with epilepsy were enrolled in the Epilepsy Center Sichuan Provincial People's Hospital from April 2015 to November 2016.The diagnosis was in line with the classification of epilepsy defined by the International league against epilepsy(ILAE,1981).Only patients with generalized or secondary generalized epilepsy were recruited into the present study.Patients with genital organic or functional lesions were excluded;patients were also required to be free of serious psychosis,diabetes,malignant neoplasm and other progressive neurological diseases.Individual treatment with OXC,LEV,or LTG was randomly assigned to 38 newly diagnosed male adult patients with epilepsy.Semen quality,sex hormones and thyroid hormones were measured before treatment and 6 months after taking the medicine.A questionnaire was administered using International Index of Erectile Function Scale(IIEF5)and Premature ejaculation diagnostic tool(PEDT)self-test to evaluate sexual function,followed by an analysis of the comparison between the treated patients and healthy volunteers(healthy controls)as well as the changes and differences between the patients themselves before and after treatment.SPSS20.0(Chicago,IL,USA)was used for statistical analysis.The data were expressed as mean ± standard deviation(x ± s)when the quantitative data were consistent with the normal distribution,and Student-Newman-Keuls or Dunnett-t test was conducted for the analysis.Kruskal-Wallis and Mann-Whitney tests were used for quantitative data that did not fit the normal distribution,and the constituent ratios of the rates were determined using the Pearson chi-square or Fisher's exact test.A difference of P < 0.05 was regarded as statistically significant.Results: 1.The total sperm count,fast forward movement rate(FFMR),survival rate,and normal sperm rate in the group with epilepsy were lower than those in healthy controls(P < 0.05).The FFMR and survival rate of sperm after OXC treatment were significantly higher than before treatment(P < 0.05).All semen parameters after LEV and LTG showed a possible trend for improvement,but no significant statistical difference(P< 0.05).2.Male erectile function score in patients with epilepsy and premature ejaculation index scale scores were lower than that of healthy controls,but there was no statistically significant difference(P > 0.05).Three different antiepileptic drugs(OXC,LEV,LTG)had a tendency to improve at erectile function before and after treatment comparison,no improvement at premature ejaculation degree,and there were no statistically significant difference(P > 0.05).3.There was no significant difference in sex hormone levels in the epilepsy group before treatment compared with the healthy controls,or when compared after treatment with the three different AEDs(P< 0.05).4.Male patients with epilepsy before taking the medicine compared with healthy controls,the thyroid hormone levels had no significant difference(P > 0.05).OXC group after 6 months,total thyroxine(TT4)and free thyroxine(FT4)significantly lower than the baseline(P < 0.05).Thyroid hormone levels in the treatment group(LEV and LTG)compared with before treatment,no statistical difference(P > 0.05).5.The marital rate and fertility rate of patients with epilepsy were significantly lower than those of healthy controls(P < 0.05).Significance: 1.The semen quality of males with epilepsy is decreased even before treatment,so the epilepsy itself may affect adult male patients with epilepsy semen quality.2.New antiepileptic drug OXC,LEV,LTG no obvious adverse effect on patients semen quality.OXC can obviously improve the fast forward movement of the sperm and the sperm survival rate,for claiming to have fertility of male patients with epilepsy OXC would be more appropriate.3.OXC can reduce serum total thyroxine(TT4)and free thyroxine(FT4),was harmful to thyroid hormone of patients.4.New antiepileptic drug OXC,LEV,LTG had no significant effect on the patients' sexual function,sex hormone.
