ObjectiveThis study was designed to investigate the prevalence and risk factors of anxiety and depression disorder in preoperative patients who came to aesthetic plastic surgery clinic and to provide some suggestion to improve the preoperative evaluation.The Hospital Anxiety and Depression Scale(HADS)was applied to analyze the anxiety and depression characteristics in these patients.MethodsUsing the field survey method,the estimate of anxiety and depression in preoperative patients was made by the Hospital Anxiety and Depression Scale from July 2016 to July 2017 in the outpatient department of plastic surgery,the First Affiliated Hospital,College of Medicine,Zhejiang University.The baseline data was collected and analyzed.All data were analyzed by SPSS(version 22.0 for windows)statistical package.ResultsFrom July 2016 to July 2017,a total of 315 patients were enrolled,including 15 males and 300 females,with an average age of 29.57 + 8.38 years.A total of 322 kinds of surgery were performed including cosmetic surgeries on eyes,eyebrows,noses,facial contour,breasts and perineum.In the 315 patients,the average HADS score was 8.22 + 5.13 and there was no significant difference between male and female scores(p=0.407).The average score of anxiety scale was 4.71±3.04 and that of depression score was 3.51±2.72.The anxiety scale score was higher than depression score with the significant difference(p=0.000).According to gender,we divided the 315 patients into 2 parts.Women's Anxiety Scale score was significantly higher than that of depression scale score(p=0.000),while male anxiety score and depression score were not significantly different(p=0.067).The scores of anxiety and depression between male and female were not significantly different(HADS-A,p=0.414;HADS-D,p=0.254).The anxiety and depression scores more than 9 points mean the anxiety and depression state may be suspicious positive.There were 34 cases(10.79%)with suspicious anxiety state and 15 cases(4.76%)with suspicious depression state and 6 cases(1.90%)with both suspicious anxiety and depression state.The baseline data was analyzed by multiple linear regression and Logistic regression.We found that education level of these patients(p=0.001)and the development level of their residence places(p=0.040)have a negative correlation with the score of anxiety.Besides,there was a negative correlation between education level(p=0.000)and depression score.There was a negative correlation between education level(p=0.000)and depression score.The education level was associated with diagnosis of anxiety(p=0.034).The education level(p=0.001)and occupations(p=0.015)were associated with the diagnosis of depression.ConclusionIn the patients seeking for aesthetic plastic surgery,to some degree,there were some anxiety and depression symptoms before they were operated.The preoperative anxiety is more obvious than depression.The gender,age,operation frequency,and the categories of operation have no significant correlation with anxiety and depression state.There is negative correlation between education level and anxiety and depressive symptoms.It is necessary to evaluate the patients' psychological state before the performance of aesthetic plastic surgery.
