Objective:Ghrelin and nesfatin-1 are newly discovered feeding regulating peptides,they play important roles in regulating appetite and energy balance.A comprehensive understanding of the ghrelin and nesfatin-1 factors associated with obesity contributes to the intensive study and prevention of childhood obesity.In addition,the incidence and severity of infectious diseases in obese children is higher than those in normal children,and the cause may be related to immune dysfunction.In this study,we detected the levels of fasting serum ghrelin,nesfatin-1,blood glucose(GLU),related indexes of blood lipid metabolism(such as cholesterol(TC),triglyceride(TG))and immune related markers(C3,C4,immunoglobulin(such as IgA,IgG,IgM))in simple obese children,anddid correlation analysis,to explore the relationship between obese children’s serum ghrelin,nesfatin-1 and BMI,the levels of blood GLU,TC,TG,C3,C4,IgA,Ig G,IgM.By comparing the differences of ghrelin and nesfatin-1 between simple obesity children and normal control children,the effect of the two on obesity was explained.Methods:From January 2013 to December 2014,49 cases of simple obesity children(simple obese group)were enrolled from the children’s health care clinic of Hebei Children’s Hospital,there were 37 males and 12 females aged 3-14 years,the average age was(9±2.69)years,after examination,other secondary diseases such as inherited metabolic diseases,endocrine diseases and central nervous system diseases were excluded,the diagnostic criteria for obesity were the overweight and obesity body mass index(BMI)classification criteria for Chinese children.In addition,37 healthy children who were examined in our hospital during the same period were selected as the normal control group,there were 26 males and 11 females,aged 3-14 years,the average age was(7.95±3.13)years,after examination,the liver,lung,kidney,endocrine and other diseases and malnutrition were excluded.There was no statistical differen-ce in age and sex between the two groups.The subjects could not exercise violently for 24 hours before the onset of blood and fasted overnight,on the next morning,taken off shoes and wore unlined garment to measure height and weight.Subjects were tested for age,sex,height,weight,blood pressure,and so on.BMI=weight(Kg)/height ~2(m~2).Fasting blood samples were taken from the subjects,double serum was collected,after serum separation,the serum GLU,TC,TG,C3,C4,IgA,IgG and IgM concentrations were measured by cobas8000 au-tomatic bioche-mical immunoassay instrument immediately.Meanwhile,en-zyme linked immunos-orbent assay(ELISA)was used to determine the c-oncentrations of ghrelin and nesfatin-1 in the other serum of the same subjects.In order to illustrate the influences of sex and age on serum ghrelin and nesfatin-1 levels,the children in simple obesity group were divided into groups according to age and sex,serum ghrelinand nesfatin-1 levels were compared between male and female children,children over 6 years old(school age)and aged 6 years and younger(preschool age).Summarized all data,statistical analysis was performed with SPSS13.0 statistical software,all data conformed to the normal distribution,the data was represented by mean±standard deviation((?)±s),the t test of completely randomized design was used between the twogroups,P<0.05(bilateral)had significantly significance.Pearson correlation test was used in the correlation analysis.Results:1 Comparison of serum ghrelin levels between groupsSerum ghrelin levels in children with simple obesity(1343.8±1150.53ng/L)are significantly lower than the normal control group(2003.5±1337.51ng/L),t=2.403,P=0.019(P<0.05),there is significantly statistical difference;Serum ghrelin levels in the male group of simple obese children(1479.7±1294.6ng/L)are significantly higher than the female group(924.7±193.5ng/L),t=2.522,P=0.016(P<0.05),there is significantly statistical difference;The serum ghrelin levels in children with simple obesity over the age of 6(1234.6±964.4ng/L)and aged 6 years and younger(1769.9±1580.6ng/L)have no statistical significance,t=1.323,P=0.192(P>0.05).2 Comparison of serum nesfatin-1 levels between groupsSerum nesfatin-1 levels in children with simple obesity nesfatin-1(492.8±416.90ng/L)are significantly lower than the normal control group(727.3±399.72ng/L),t=2.628,P=0.010(P<0.05),there is significantly statistical difference;Serum nesfatin-1 levels in the male group of simple obese children(549.9±465.3ng/L)are significantly higher than the female group(316.9±71.1ng/L),t=2.942,P=0.005(P<0.01),there is significantly statistical difference;The serum nesfatin-1 levels in children with simple obesity over the age of 6(459.3±366.4ng/L)and aged 6 years and younger(623.4±537.8ng/L)have no statistical significance,t=1.113,P=0.271(P>0.05).3 Correlation between serum ghrelin levels and detection indexes in children with simple obesityThe level of serum ghrelin in children with simple obesity group are significantly correlated with BMI(r=0.823,P=0.000),but not correlated with GLU,TC,TG,C3,C4,IgA,Ig G and Ig M.4 Correlation between serum nesfatin-1 levels and detection indexes in children with simple obesityThe level of serum nesfatin-1 in children with simple obesity group are significantly correlated with BMI(r=0.792,P=0.000),but not correlated with GLU,TC,TG,C3,C4,IgA,gG and IgM.Conclusions:1 The serum levels of ghrelin and nesfatin-1 in the simple obesity group are significantly lower than those in the normal control group,these findings suggest that ghrelin and nesfatin-1 play an important role in the pathogenesis of childhood obesity.2 The serum levels of ghrelin and nesfatin-1 in the male group of obese children are significantly higher than those in the simple obese women group,It is speculated that the secretion of ghrelin and nesfatin-1 be related to Gender.3 Ghrelin and nesfatin-1 are significantly related to BMI,It suggests that ghrelin and nesfatin-1 are closely related to obesity,Moreover,serum levels of ghrelin and nesfatin-1 are significant factors affecting BMI in simple obesity patients.4 There is no correlation between ghrelin and nesfatin-1 and GLU,TC,TG,C3,C4,IgA,Ig G and IgM,Therefore,ghrelin and nesfatin-1 have not affected the blood glucose and blood lipid levels and immune function.
