Construction And Application Of Chemotherapy Protective Scheme For Nurses In Non-oncology Department Based On Action Research Pattern

The first partObjective To understand the current situation for nurses in non-oncology department in third-grade class-A hospital in YinChuan.Based on action research pattern and combined with the current situation of chemotherapy protection,the scheme of chemotherapy protection was constructed.Methods Using the facilitating sampling method,332 nurses in non-oncology department in third-grade class-A hospital in Yinchuan were investigated by general data questionnaire,the questionnaire of knowledge,attitude and practice of chemotherapy protection and the scale of hospital safety climate.The level of knowledge,attitude,practice and its influencing factors of chemotherapy protection was analyzed.The pattern of action research was adopted to identify the problems of chemotherapy protection,the scheme of chemotherapy protection was constructed.Then,according to the cyclical process of plan-act-observe-reflect,the existing problems in scheme was analyzed.Thus,the measures were put forward until the optimized scheme of chemotherapy protection was formed.Results(1)The score of chemotherapy knowledge of nurses in non-oncology department was(18.40±3.49),the score of chemotherapy attitude was(40.83±4.13),the score of chemotherapy practice was(40.83±4.13).(2)The score of hospital safety climate was(74.18±4.01),the lowest score was 46 points,while the highest was 85 points which was at a medium level.(3)Correlation analysis showed: the score of knowledge,attitude,practice of chemotherapy protection and the total score of KAP was positively correlated with training education,working environment,protective equipment and manager's attitude(P<0.01).(4)Single factor analysis showed: the KAP scores of chemotherapy protection had statistical difference in the terms of age,positional title,education,department,working year,marriage and types of protective appliance(P<0.05).(5)Multiple step-wise regression analysis showed: training education,protective equipment,manager's attitude,working environment,working year and positional title were the major factors influencing the KAP scores of chemotherapy protection of nurses in non-oncology department.(6)After using action research pattern to construct the scheme of nurse's chemotherapy protection,paired t-test analysis showed that the KAP scores of nurse's chemotherapy protection was significantly higher than that before intervention(P<0.05).(7)After using action research pattern to construct the scheme of nurse's chemotherapy protection,paired t-test analysis showed that the score of each dimension and the total score of hospital safety climate was significantly higher than that before intervention(P<0.05).The second partObjective In view of the problems existing in the chemotherapy protection,combined with the scheme,the effect of intervention about chemotherapy protection was studied.Method Using the cluster sampling method,105 nurses in non-oncology department were selected.They were randomly divided into control group and intervention group.The nurses in control group were given routine study,while the nurses in the intervention group were given scheme about chemotherapy protection.Before and after intervention,the level of knowledge,attitude,practice and hospital safety climate were compared respectively and the effect of the scheme was evaluated.Results(1)The chi-square test was used to compare the baseline data of the two groups of nurses.There was no significant difference in sex,age,positional title,education and so on.(P>0.05).(2)After the intervention of chemotherapy protection,two independent sample t-test showed: the KAP scores of chemotherapy protection was significantly higher than that in the control group(P<0.05).(3)After the intervention of chemotherapy protection,two independent sample t-test showed: the score of each dimension and the total score of hospital safety climate was significantly higher than that in the control group(P<0.05).Conclusions1.The nurses in non-oncology department lacked the knowledge of chemotherapy protection,especially mechanism and side effects of chemotherapeutic drugs.Behavior and attitude of chemotherapy protection are also need to be further improved.Training education,protective equipment,manager's attitude,working environment,working year and positional title were the major factors influencing the KAP scores of chemotherapy protection of nurses in non-oncology department.Then again,the score of each dimension and the total score of hospital safety climate was low,which was at a medium level.Because of existing problems and influencing factors,it is very important to construct the scheme of chemotherapy protection and train nurses in non-oncology department.2.The effect of intervention showed: After using action research pattern to construct the scheme of nurse's chemotherapy protection,the level of knowledge,attitude,practice was improved significantly.Then again,the score of hospital safety climate was improved significantly.It is suggested that constructing the scheme of chemotherapy protection based on action research pattern can improve the effect of chemotherapy protection.
