Due to the limitation of social security, our country's old-age pension system still mainly depends on family pensions. On the basis of summarizing the previous experience of retirement and the successful experience of the home-based care model for foreign communities, the Chinese government has proposed the dominant form of community home-based care. However, for the community-home social pension model, the intervention and participation of social forces from professional organizations such as social work and social organizations are indispensable. At present, although Chinese scholars are constantly exploring the feasibility and necessity of social organizations involved in community-based home-based care for the elderly, and the paths and methods of intervention, they have been obtained in the process of social work agencies involved in home-based care services. There are few studies on the effectiveness and problems that emerged, and there are fewer studies to analyze the cases. This paper summarizes the case studies of KLJ social worker institutions involved in home-based care services in the R-street of S city on the basis of summarizing the domestic and foreign relevant social work organizations involved in community home-based care service model research, and reveals the case in which our social-social organization is involved in urban community home-based care services. To its own positioning is not accurate, effective integration of resources and other issues. The study believes that effective measures must be taken to solve the problems such as the effective integration of resources actually existing in community home-based care, service content and demand equivalence, and the accurate positioning of social worker institutions themselves. This will provide an effective institutional basis for social workers' intervention in home-based care and policy support.
