Study On The Difference Of Suicidal Ideation Between Urban And Rural Elderly In Shandong Province

BackgroundSuicide has been the major public health problem in the world,which leading to huge economic and social problems,even also for individuals,families,communities and countries,causing heavy burden.The elderly has a high suicide rate,and suicide rate is highest in the elderly over the age of 75 years,and the aging process is fast and deep in our country,the elderly population in our country is huge,so the study of the suicidal behavior is necessary.And suicidal ideation is an inevitable stage,which is suicide,suicide attempts and completed suicide risk factors,so the study of the elderly suicide ideation is very important.Despite the elderly suicide ideation developed at home and abroad,it is mainly to the study of elderly at abroad.And at the same time,there is little study of suicide ideation among the urban and rural elderly.ObjectiveThe study in order to understand the incidence of suicide ideation among the urban and rural elderly,analysis the influence factors of suicide ideation in urban and rural elderly,providing the basis for formulation and reducing the generation of the elderly suicide rate,so as to improve the elderly’s life and life quality.MethodsThis study adopts the method of multistage stratified sampling,27 cities and 27 rural communities in Shandong province has been selected and surveyed 3416 elderly.We designed the questionnaire and used the method of face to face to complete the survey.We used Epidata software and SPSS software for data entry and analysis.Through descriptive analysis to explore the basic situation of suicidal ideation in the elderly,logistic regression and chi-square test were used to compare the statistical relationship between the various influencing factors and the suicidal ideation of the elderly in urban and rural areas.According to the single factor analysis,two models were established to explore the mediating relationship between the initial and dependent variables.Based on the multi-factor analysis results,the structure equation model was established and the path analysis diagram was made.ResultsThis study found that the rate of suicidal ideation was 5.2%in rural elderly,2.5%in urban areas which was lower than rural areas.The total effect value and the direct effect value of depression on suicidal ideation in the rural elderly were both β=0.718(p<0.05).The total effect value of the economic self-assessment was β=-0.233(p<0.05),and the indirect effect value was β=-0.253(p<0.05).The total effect value of the number of chronic diseases was β =0.096(p<0.05),and the indirect effect value was β=0.142(p<0.05).Life satisfaction of the total effect value was β=0.169(p<0.05),direct effect value was β = 0.067(p<0.05),the indirect effect value was β =0.236(p<0.05).The total effect value and the direct effect value of depression on suicidal ideation in the urban elderly were both was P=0.625(p<0.05).The total effect of gender value was β = 0.514(p<0.05),direct effect value was β = 0.402(p<0.05),the indirect effect value was β = 0.113(p<0.05);The total effect value of the number of chronic diseases was β=0.168(p<0.05),and the indirect effect value wasβ=0.098(p<0.05).The total effect value of social support was β = 0.173(p<0.05),the indirect effect value was β = 0.205(p<0.05).Conclusion and suggestionConclusion1.There is a high incidence of suicidal ideation among the elderly in urban and rural areas in Shandong province,which the rural area is significantly higher than that of the city.2.The depression has the greatest direct effect,the total effect on suicide ideation in urban and rural elderly.And self-rated economic status of the total effect in the rural elderly is inferior to depression,self-reported life satisfaction follow to economy.The top three factors among urban and rural elderly people were depression,gender and social support.3.Economic self-assessment,the number of chronic diseases and life satisfaction are the risk factors that affect the suicidal ideation of the elderly in rural areas;Gender,education,number of chronic diseases and social support are the factors that influence the suicidal ideation of urban elderly.Suggestion1.Pay great attention to the mental health of the elderly and do a good job in mental health,establish a network of mental health counseling for the elderly,and provide early screening and early treatment for depression among the elderly in the community.2.Comprehensively publicize and raise the awareness of the elderly on suicide,and promote the ability of the elderly to adapt to the surrounding environment through the promotion of education to the elderly by community-based education.3.Improve the social old-age security system and improve the level of medical care,so as to reduce the pressure of life among the elderly.4.Improve the social support of the elderly,construct a harmonious family ethics atmosphere,and promote the excellent traditional virtue of respecting and respecting the old.
