Construction Of Evaluation Index System Of Comprehensive Ability Of County-level Public Hospitals

ObjectiveThis study aims to construct a comprehensive evaluation index system for county-level public hospitals in the context of “Health China 2030” to accurately and objectively understand the key factors that reflect the core competence of county-level public hospitals and evaluate the comprehensive capacity of county-level public hospitals.The index system is used to strengthen the construction of county-level public hospitals,improve the level of medical services at county-level public hospitals,strengthen the awareness of quality,allocate health resources efficiently,and provide scientific basis for county-level public hospitals and related departments to carry out evaluation work and formulate reform and development strategies.MethodsOn the basis of domestic and foreign literature review and government policy review,through thematic group discussions and brainstorming methods,the county-level public hospitals comprehensive ability evaluation index system was initially formulated,and an expert consultation questionnaire was formed;twenty experts from the health administration,universities,hospitals and other fields were selected to conduct two rounds of expert consultations and finalize the comprehensive evaluation index system for county-level public hospitals.Based on this index system,the Analytic Hierarchy Process(AHP)was used to determine the weights of indicators at all levels.Results1.the results of Delphi method expert consultation: 1 basic information of experts: a total of 30 experts in related fields were included for consultation,including 24 masters(80.0%)and senior professional titles(76.7%);2 experts positive degree: the effective questionnaires recovery rate of the two rounds of expert consultation questionnaires were 88.2% and 100%,and the degree of positiveness was good.3 the degree of authority of the experts: the degree of authority of the experts was all greater than 0.7;4 coordination of expert opinion: in the first round of expert opinions,the coordination coefficients of the importance of indicators at the various levels of were 0.308,0.200,and 0.221,respectively;the coordination coefficients for the importance of the indicators of the second round of expert opinions at various levels were 0.297,0.266,and 0.218,respectively.The results of the chi-square test of the coordination coefficient were Statistically significant(P<0.05).2.The comprehensive evaluation index system for county-level public hospitals was constructed,including 6 primary indicators,15 secondary indicators and 62 tertiary indicators.The comprehensive evaluation index system was comprised by medical resources,medical services,hospital management,science and education and innovation,public welfare,and satisfaction.3.The weight coefficients of the first,second,and third-level indicators was determined.The first-level indicators includes medical resources,medical services,hospital management,science,education and innovation,public welfare,and satisfaction.The weights of those indicators were 0.1606,0.3512,0.1795,0.1513,0.0719 and 0.0854,respectively.Conclusions1.This study constructs a comprehensive evaluation index system for county-level public hospitals in line with the requirements for the development of “Health China 2030”.In this index system,6 first-level indicators including medical resources,medical services,hospital management,science,education and innovation,public welfare,and satisfaction,and 15 second-level indicators,and 62 third-level indicators were included.And the corresponding weight coefficient of the indicators at all levels were determined.With tight internal logic,the evaluation index system has a scientific and rational structure,and strong scientificity,practicability,and operability.2.On the basis of the construction of scientific evaluation index system,and in order to improve the comprehensive capacity of county-level public hospitals,this study proposes the following suggestions: strengthening the talent team construction,improving the medical services levels and the modern management levels,and taking into account both public welfare and social satisfaction.
