| Background:Public health emergencies have caused huge losses to the state and society,seriously threatening the safety of people’s lives and property,social stability and harmony,economic construction and sustainable development.In the emergency handling of public health emergencies,public health emergency resources are the premise and foundation,a large number of emergency medical resources require rapid supply and allocation in a short period of time,and how to allocate them is directly related to the timeliness and effectiveness of incident handling.Henan Province is a large agricultural province,the population density is high,the flow of personnel is large,70% of the population gathered in the county,and the county-level public hospital emergency response capacity has shortcomings,can not be reasonable and effective allocation of medical and health emergency resources,but at present Henan Province county-level public hospital public health emergency resource allocation capacity What problems exist,capacity evaluation should be carried out from what aspects and how to evaluate,there is no scientific specific evaluation system,so the development of a systematic,comprehensive,Practical and feasible emergency resource allocation capacity evaluation system,it is necessary to evaluate the emergency resource allocation capacity of public health incidents in county-level public hospitals in Henan Province,which can not only test the development of public health emergency resource allocation capacity of county-level public hospitals in Henan Province,but also provide a reference basis for improving the emergency resource allocation capacity of county-level public hospitals in Henan Province and improving the public health emergency management system of county-level medical and health institutions in Henan Province.Objectives:Taking the county-level public hospitals in Henan Province as the evaluation unit,based on the relevant theories of emergency management and resource allocation,drawing on the existing research methods for assessing the response capacity of public health emergencies at home and abroad,constructing an evaluation index system for the allocation capacity of public health emergency resources in county-level public hospitals,and on this basis,developing an evaluation questionnaire for the allocation capacity of public health emergency resources in line with the actual situation of county-level public hospitals in Henan Province,and applying it to evaluate the current situation of public health emergency resource allocation in county-level public hospitals in Henan Province.Therefore,it puts forward policy suggestions for strengthening the capacity building of public health emergency resource allocation in county-level public hospitals in Henan Province,and provides a reference for improving the emergency level of public health emergencies in county-level medical institutions in Henan Province.Methods:1.Literature review: Based on literature review,relevant literature review and policy review were carried out to determine the core elements of emergency resource allocation ability of county-level general hospitals and establish the initial framework of evaluation index system based on the theories of emergency management and resource allocation.2.Delphi expert consultation method: invite experts in related fields to carry out two rounds of expert consultation on the initial framework of the evaluation index system of emergency resource allocation capacity,and form the final evaluation index system of public health emergency resource allocation capacity of county-level public general hospitals;3.Analytic Hierarchy Process: use analytic hierarchy process to determine the dimensions of the evaluation index system of emergency resource allocation capacity and the weight of each index;4.Questionnaire survey method: According to the determined evaluation index system,a questionnaire was prepared for the public health emergency resource allocation capacity of county-level public general hospitals,and an empirical study was conducted on the emergency capacity of some district and county public general hospitals in Henan Province by using the questionnaire survey method.Results:1.The core elements of the public health emergency resource allocation capacity of county-level public hospitals were sorted out,including 9 main elements and 41 corresponding evaluation contents such as the number of emergency personnel,personnel structure,emergency quality of personnel,material quality,material quantity,capital and information guarantee.2.The evaluation index system of public health resource allocation capacity of county-level public hospitals was determined.Based on the theory of emergency management and the theory of resource allocation,the evaluation index system of public health emergency resource allocation capacity of county-level public hospitals is proposed and constructed,and the evaluation index system and its weight are determined by two rounds of expert consultation method and analysis at level,including 4 first-level indicators,9 second-level indicators,and 39 third-level indicators,all of which are within a reasonable range after the reliability test.3.An empirical study on the evaluation model of public health resource allocation capacity of county-level public hospitals: The results show that the comprehensive average score of emergency resource allocation capacity of county-level public hospitals is 748.52 points,the highest score is 822.45 points,and the lowest score is 653.52 points.The draw score of the first-level indicators is: emergency manpower support 0.767 points,emergency material support 0.795 points,emergency fund support 0.732 points,emergency information support 0.617 points.4.Put forward policy suggestions to enhance the capacity of public health emergency resources allocation in county-level public general hospitals: first,increase the intensity of emergency drill training and enrich the content and types of drill training;Second,improve the types of emergency supplies such as medicines;The third is to reduce the pressure of medical insurance fund by establishing special emergency fund for medical insurance,and give full play to the role of medical insurance fund in emergency response to public health emergencies.Fourthly,strengthen the information construction of the hospital and smooth the emergency information sharing channel with other medical and health institutions in the county.Conclusions:The evaluation system of public health emergency resource allocation capacity of county-level public hospitals constructed by this institute has high reliability and rationality,and can be used as the basis and reference for comprehensive evaluation of the emergency resource allocation capacity of county-level general hospitals.The empirical research results show that the capacity of public health emergency resource allocation in county-level public hospitals in Henan Province needs to be further improved,and the training exercises for emergency personnel,the completeness of emergency material types,the balance and utilization of medical insurance funds,and the sharing of emergency information still need to be improved and strengthened.Due to the limitations of various conditions,the number of experts in public health emergency management consulted by this study is not enough,and the empirical research is only carried out in 6 county-level general hospitals in three prefecture-level cities in Henan Province,for the future research on the allocation of emergency resources,the next step will be to increase the number of authoritative experts in related fields such as emergency management and public health,further expand the scope of the survey and the number of county-level public hospitals,and further improve the resource allocation evaluation index system and questionnaire on the basis of the current research.The sample hospitals will be promoted to public hospitals in the county across the country. |
