Objective:The purpose of this study was to construct the BCMabl-MnPs complex,and detect the specific identification of BCMabl-MnPs complex detecting tumor cells in urine of patients with bladder urothelial carcinoma(BUC).Then evaluate the clinical application value of BCMabl-MnPs in bladder urothelial carcinoma early diagnosis.Methods:1.The indirect coupling method was adopted to were constructed by binding the monoclonal antibody BCMabl in Magnetic beads,when the bladder urothelial carcinoma immunomagnetic beads was prepared,mixing the bladder urothelial carcinoma EJ cells and normal bladder epithelial cells HCV29 suspension,then this two cell populations will be separated by Immunological magnetic force.Further the immunocytochemistry method was performed to identity the Specificity and sensibility of immunomagnetic beads detection method.2.The duration of 2016 June to 2018 March patients suspected for bladder cancer in the second affiliated hospital of Kunming medical university were selected 71 patients diagnosed as bladder urothelial carcinoma with Cystoscopy and Postoperative pathological diagnosis were set as tumor group,and 20 non-urological tumor patients were included as control group.NMP22?BTA?BCMab1 and traditional urine cytology methods were conducted to detect the urine samples in both two groups.Statistical Difference of Specificity and sensibility of these four methods were analyzed by SPSS19.0.then the clinical application of BCMabl-MnPs could be evaluated Preliminarily.Results:1.BCMabl were constructed by binding immunomagnetic beads could sensitively and exclusively combined with EJ cells.When the ratio of EJ cells and normal bladder epithelial cells was 5×105:1,the tumor cells could be detected.2.In comparison with other 3 methods(NMP22?BTA?and traditional urine cytology method,BCMabl-MnPs benefits the highest sensibility(92.96%)and Specificity(100%)in bladder urothelial carcinoma diagnosis through urine detection.3.The sensibility of BCMabl-MnPs method in detecting low grade bladder urothelial carcinoma was 76.47%,as apposed to in high grade bladder urothelial carcinoma(98.18%),and a significant Statistical difference was found in this two group(p=0.009).4.There was a marked distinction in sensibility of NMP22 and traditional urine cytology methods to diagnose different Pathological staging,and the diagnosis sensibility of these two detection in>T3 patients was superior to<T3patients.5.The BTA concentration in urine was positive related to Pathological grading and staging,and statistical differences were found in the analysis outcomes.6.The sensibility and specificity of NMP22?BTA and BCMabl-MnPs combined detection method could reached to 95.77%and 85%respectively.Conclusions:BCMabl-MnPs is an novel bladder urothelial carcinoma diagnosis method with high sensibility and specificity,and will be a promising urine cytology detection applicated in bladder urothelial carcinoma early diagnosis.
