Objective:To evaluate and compare the psychological status and quality of patients with early and advanced breast cancer,and to evaluate the intervention effect of psychological intervention on the psychological status and quality of life of patients with advanced breast cancer.Methods:This study collected 250 cases of breast cancer patients hospitalized in the Department of Oncology of Tumor Center of the First Hospital of Jilin University from August 2016 to December 2017,including 100 patients with early stage and 150 patients with advanced stage.The psychological status and quality of life of the patients were evaluated by Self-Rating Anxiety Scale(SAS),Self-rating depression scale(SDS),European Organization for Research and Treatment Quality of Life Questionnare-Core30(EORTC QLQ-C30).The 150 patients with advanced breast cancer were divided into the intervention group(75 cases)and the control group(75 cases).The intervention group was given routine treatment and psychological intervention(including psychological education,group psychological intervention,supportive intervention,and social support intervention),and the control group was given routine treatment only.Results1.The anxiety scores of early and advanced breast cancer patients were 43.42±8.54 points and 45.57±8.16 points,and the ratios were 23% and 36% respectively.Among them,the mild anxiety rates were 9% and 25.3% respectively,and the moderate anxiety rates were 12% and 8.7% respectively,and the severe anxiety rates were 2% and 2% respectively.The depression scores were 50.34±10.36 points and 49.56±9.87 points,respectively.The ratios were 42% and 43.3% respectively,of which mild depressive rates were 22% and 26% respectively,and moderate depression rates were 10% and 14.7% respectively,and the severe depressive rates were 2% and 2.6%,respectively.The anxiety scores and rates of patients with advanced stage were significantly higher than those in the early stage(P <0.05),and there was no significant difference in depression scores and ratios between early and late stage patients(P>0.05).2.Somatic function and role function score,social function,overall health status,fatigue,insomnia,loss of appetite were significantly different between the late stage patients and the early stage patients(P<0.05),indicating that the patients with advanced stage had more serious problems in the above aspects.3.The intervention group and control group had no statistically significant differences(P>0.05)in age,marriage,occupation,whether medical expenses at their own expense,recurrence and metastasis of breast cancer,number of recurrence and metastasis,and first-line treatment options(P>0.05).There were no statistically significant differences in the scores of anxiety,depression scores and ratios,and quality of life(P>0.05),which were comparable.After intervention,the anxiety scores of the intervention group and the control group were 41.63 ±8.73 and 45.57 ±7.59,respectively,and the ratios were 18.7% and 33.3% respectively.The decrease of anxiety score in the intervention group was significantly lower than that in the control group(P<0.05).4.After intervention,the scores of physical function,role function,emotional function,cognitive function,and overall health of the intervention group were higher than those of the control group.The scores of fatigue and insomnia symptoms were lower than those of the control group,and the difference was statistically significant(P<0.05),indicating that the above dimensions improved after intervention,and there was no statistical difference in the remaining dimensions Conclusion:1.There were psychological problems of anxiety and depression in breast cancer patients,and mainly mild to moderate.The degree and rates of anxiety in patients with advanced breast cancer were higher than those in patients with early stage,and there was no significant difference in the degree and rates of depression.2.The quality of life of breast cancer patients was at a moderate level,and the functional field status of advanced patients was poor,and the symptoms areas were characterized by fatigue,Insomnia and loss of appetite.3.Psychological intervention could improve the psychological problems of anxiety and depression in patients with advanced breast cancer,and it was more effective for patients with mild anxiety and depression.4.The quality of life of advanced breast cancer patients given psychological intervention was improved.
