Effect And Clinical Significance Of Different Blood Purification Methods On Microinflammatory Indicators In Patients With End-stage Renal Disease

Objective: To observe the changes of microinflammatory indicators in patients with end-stage nephropathy,research the influencing factors of microinflammatory indicators in patients under different blood purification schemes and put forward the intervention measures to provide reference basis for alleviating the microinflammatory indicators of patients.Method: Patients undergoing hemoblood purification treatment in xiangxiang people’s Hospital in May and June 2017 were be selected,and patients’ general conditions and blood purification related data were be accessed from medical records.The fasting upper extremity venous blood was extracted to determinate the C-reactive protein(CRP),Interleukin-6(IL-6),Tumor Necrosis Factor-α(TNF-α),Total Protein(TP),Serum albumin(ALB),Transferring siderophilin(TRF),Hemoglobin(Hb),Calcium(Ca),Sodium(Na),Potassium(K),Phosphate(P),Urea nitrogen(BUN),Uric Acid(UA),Serum creatinine(Scr),Parathyroid hormone(PTH),Cystatin C(CysC),β2-microglobulin(β2-MG)from patients before and 3 months,6 months after the blood purification.All data were inputted into Epidata system and analyzed by SPSS 20.0 statistical software.Result:(1)There were significant differences of CRP,IL-6 and TNF-α among different time points(P<0.001),there were significant differences of CRP(P=0.046)and IL-6(P < 0.001)among different blood purification methods,the change trend of CRP,IL-6 and TNF-α(P < 0.001)with different blood purification methods were significant different;(2)There were significant differences of TP,ALB,TRF and Hb among different time points(P<0.001),there were significant differences of Hb(P=0.023)among different blood purification methods,the change trend of ALB,TRF and Hb(P < 0.001)with different blood purification methods were significant different;(3)Except TP with IL-6 and TPN-α(3 months after blood purification),TNF-α and TP(6 months after blood purification),there were negative correlations between other indicators of microinflammatory and nutrition(P<0.05);(4)Blood purification mode(P=0.020)and β2-MG(P<0.001)have an effect on CRP level after blood purification,blood purification mode(P<0.001),CysC(P=0.047)and β2-MG(P<0.001)have an effect on IL-6 level after blood purification,blood purification age(P=0.005)and β2-MG(P<0.001)have an effect on TPN-α level after blood purification,that is,blood purification mode,blood purification age,CysC and β 2-MG levels have difference influence on microinflammatory indicators.Conclusion: high permeability hemodiafiltration could effectively alleviate the indicators of microinflammatory.Furthermore,blood purification mode,blood purification age,CysC and β2-MG levels have different effect on microinflammatory indicators.
