Objective:Using the function of 256-layer iCT in the determination of cardiac function and arterial elasticity index,the arterial elasticity,left ventricular function and their correlation were compared between patients with aortic dissection and non-dissected controls,and to provide a reference for the clinical treatment of patients with aortic dissection.Methods:This study selected 40 cases of chest,back or abdominal pain to our hospital emergency,clinical symptoms were highly suggestive of aortic dissection,plain or enhanced diagnosis aortic dissection in patients as a case group,At the same time,randomly selected 40 cases without aortic dissection as the control group.All subjects underwent CTA one-stop scanning and measurement of elastic and cardiac function at the same level.According to the measurement results,the arterial elasticity indexes of the case group and the control group were evaluated and compared with the measured cardiac function to evaluate the arterial elasticity and its correlation with left ventricular function in patients with aortic dissection.To discuss the arterial elasticity and cardiac function differences between Stanford A and tanford B dissection patients;to discuss the effect of age on arterial elasticity and cardiac function in aortic dissection patients and normal people.Results:The degree of arterial expansion(AoD)and compliance(AoC)in patients with aortic dissection were significantly lower than those in the control group(P<0.05).There was no significant difference in the mean diameter change rate(%Ao)and stiffness(AoSI)between the aortic dissection patients and the control group(P>0.05).The average AoSI in patients with aortic dissection was significantly higher than those in non-aortic dissection.There was no clear correlation between%Ao and AoC in Stanford type A aortic dissection group(r<0.3,P>0.05).%Ao,AoD,AoC,aortic diameter change(Ds-Dd),aortic area change(Ss-Sd)were highly positively correlated with each other(r>0.6,P<0.05).All indicators were negatively correlated with the AoSI of the dissection arteries(r>0.6,P<0.05),but there was no significant correlation between all indices evaluating arterial elasticity and cardiac function(EF values)(r<0.3,P>0.05).The positive correlations between%Ao,AoD,AoC,Ds-Dd,and Ss-Sd of Stanford B dissection were positively correlated with each other(r>0.7,P<0.05),and the indicators were negatively correlated with AoSI(r>0.7,P<0.05).There was also no correlation between each index and heart function(EF value)(r<0.3,P>0.05).In the control group A,there was a high negative correlation between AoD and AoC,and have a high negative correlation between%Ao and Ds-Dd(r>0.6,P<0.05),and there was a high negative correlation between%Ao、Ds-Dd with AoSI(r>0.7,P<0.05),there was no clear correlation between the other indicators(r<0.3,P>0.05),and there was no correlation between each index and heart function(EF value)(r<0.3,P>0.05).In the control group B,there was a high negative correlation between the arterial elastic AoD and AoC,between the aorta Ds-Dd and Ss-Sd(r>0.7,P<0.05),and there was a high negative correlation between%Ao,AoD and AoSI.(r>0.7,P<0.05),there was no clear correlation between other indicators(r<0.3,P>0.05),and there was no correlation between each index and heart function(EF value)(r<0.3,P>0.05).Comparing the%Ao,AoD,AoC,AoSI,left ventricular function(EF)between Stanford A dissection patients and Stanford B dissection patients,the difference was not statistically significant(P>0.05).Compared with control group A and control group B,%Ao,AoD,AoC,AoSI,left heart function(EF value),there was no statistical difference(P>0.05).The%Ao in the control group A was highly negatively correlated with age(r =-0.820,P<0.001**,)and was highly negatively correlated with AoSI(r = 0.646,P = 0.007**).At the same time,statistics showed that the ages of Stanford A sandwich group,Stanford B sandwich group,and control group B had no clear correlation with each arterial elastic index and heart function(r<0.5,P<0.05).Conclusion:MSCT was feasible and accurate in assessing arterial elasticity and left ventricular function in the aortic dissection.Within 24 hours,the preoperative aortic dissection arteries with reduced distensibility and compliance and increased stiffness are important signs of arteriosclerosis.There was a significant correlation between the elastic indices of the arteries in the aortic dissection and they had mutual influence.There was no correlation between age and cardiac function with elasticity of aortic dissection.
