Analysis For Causes Of Hospitalized Children’s Iron Deficiency Anemia

Objective: To know common causes and features of hospitalized children ’ s iron deficiency anemia in different age groups,to provide refrence of formulating prevention and treatment measures.Methods: A retrospective study of clinical manifestation involving305 iron deficiency anemia hospitalized patients because of anemia aged0—16 years old in hematology department of Children’s hospital of Chongqing medical university from January 2012 to April 2017 was carried out.Results:1.In the study,moderate anemia and severe anemia were the most common,and there was no relationship between age group and anemia degree(P<0.05).2.IDA patients whose age were from 6 months to 3 years old accounted for 68.2%.The sex ratio for these 305 cases was 2.7:1,and the sex distribution of each group was different(P<0.05).3.Congenital deficient of iron storage was the most common cause in IDA patients whose age were from 0 to 6 months(81.6%),and 18.4% IDAchildren because of exclusive breastfeeding and overgrowth without iron supplement.The youngest age in 18.4% was 3.52 months.Inadequate intake of iron was the most common cause in IDA patients whose age were from 6months to 3 years old(78.4%).Excessive iron loss due to blood loss was the most common cause in 3-year-old to 16-year-old IDA patients(81.4%),while digestive tract bleeding(72.9%)and menometrorrhagia(18.8%)were the most.4.The causes of iron deficiency anemia,including deficiency congenital storage iron,insufficient iron intake,combined infection,diarrhea,suffered intestinal surgery,excessive iron loss,were different in different age groups.5.Deficient congenital storage iron(OR:3.796,95%CI:1.506-9.567)would influence the constituent of IDA in 0 to 6 month;improper feeding(OR:6.211,95%CI:3.279-11.764)would influence the constituent of IDA in 6 months to 3 years old;excessive iron loss(OR:32.444,95%CI :11.413-92.230),suffered intestinal surgery(OR : 18.214,95%CI :1.227-270.376)would influence the constituent of IDA in 3 years old to 16 years old.6.Diarrhea and deficient congenital storage iron were related with the degree of anemia,while insufficient iron intake,excessive iron loss,respiratory tract infection were not.Multivariate logistic regression analysis showed that deficient congenital storage iron(OR : 2.784,95%CI :1.659-4.672)and excessive iron loss(OR:2.201,95%CI:1.067-3.828)were the main risk factors to the severe anemia.Conclusion: The predilection age of IDA in childhood is 6 months old to 1 years old,and 3 months old to 3 years old were also the common age of IDA.The sex distribution of each group was different.The causes of iron deficiency anemia were different in different age groups,and measures should be taken to screen in different age groups according to causes.We also advice continued breastfeeding and preventive iron supplementation,especially these children who sufferer deficiency congenital storage iron and exclusive breastfeeding with overgrowth.Feeding education in necessary and we should help parents to get right knowledgement for adding supplementary food correctly and timely.Early screening of anaemia in children with diarrhea,congenital storage of iron deficiency and excessive iron loss can prevent progression of anemia.
