| Objective: To explore the therapeutical effect and its relevant mechanism of Danzhi Xiaoyao Pill to primary elderly hypertensive patients with depression and to pave a way for treating primary elderly hypertensive patients with depression through combining application of traditional Chinese and Western Medicine.Method: This study were collected a total of 120 cases of primary elderly hypertensive patients with depression in clinic and ward of integrated traditional Chinese and western medicine in Yantai Yu Huangding hospital,and randomly divided into two groups: control group(n=60)and treatment group(n=60),the control group were given antihypertensive drugs only,while those in treatment group were additionally given Danzhi Xiaoyao Pill as control.Before and 4 weeks,8 weeks after the treatment,the ambulatory blood pressure,PHQ-9 scale score,renal and liver function,and 4 kinds of cytokines CRP,AngⅡ,IL-1β,ICAM-1 were measured in two groups.Results:(1)After 8 weeks of treatment,the effective rate of treatment group was91.2%;the effective rate of control group was 76.7%,the difference between the two is statistically significant(P<0.05);(2)The ambulatory blood pressure contrast before and after treatment: after 4 weeks of treatment,the systolic pressure of treatment group was(142±7)mmHg,the systolic pressure of control group was(147±11)mmHg;the diastolic pressure of treatment group was(74±8)mmHg,the diastolic pressure of control group was(79±6)mmHg;after 8weeks of treatment,the systolic pressure of treatment group was(126±6)mmHg,the systolic pressure of control group was(135±10)mmHg;the diastolic pressure of treatment group was(65±8)mm Hg,the diastolic pressure of control group was(72±9)mmHg.The difference between the above in pairs is statistically significant(P<0.05);(3)After 4weeks,8weeks of treatment,the PHQ-9 scale score of treatment group were(8±4)points and(5±3)points while the control group were(10±2)points and(9±3)points,the difference between the two is statistically significant(P<0.05);(4)After 4weeks,8weeks of treatment,the levels of CRP in treatment group were(5.5±2.5)mg/L、(3.0±1.6)mg/L,they were declined compared with control group(6.1±2.7)mg/L、(5.8±2.6)mg/L in the same period,the difference between the two is statistically significant(P<0.05);the levels of AngⅡ in treatment group were(182.1±65.6)ng/L、(120.3±31.4)ng/L,they were declined compared with control group(181.1±71.2)ng/L、(164.7±54.9)ng/L in the same period,the difference between the two is statistically significant(P<0.05);the levels of IL-1β in treatment group were(2.77±0.70)ng/L、(1.70±0.63)ng/L,they were declined compared with control group(3.2±1.2)ng/L、(3.0±1.0)ng/L in the same period,the difference between the two is statistically significant(P<0.05);the levels of ICAM-1 in treatment group were(208.0±44.3)ng/L、(138.4±29.3)ng/L,they were declined compared with control group(248.6±101.1)ng/L、(212.6±95.8)ng/L in the same period,the difference between the two is statistically significant(P<0.05);(5)There was no obvious change between the levels of liver function(ALT,AST)and renal function(Cr)in two groups.There is no drug side effects.Conclusion:(1)The pathogenesis of primary elderly hypertensive patients with depression may be associated with the 4 kinds of cytokines,including CRP,AngⅡ,IL-1βand ICAM-1;(2)Treatment of antihypertensive drugs with pure western medicine for primary elderly hypertensive patients has poor curative effect.(3)Danzhi Xiaoyao Pill may achieve the goal of controlling the blood pression and improve the depressive state through regulating the cytokines which related to the two disease.The combination of traditional Chinese and Western medicine may open up new ideas for the treatment of this disease. |
