| Objective:In this study,on the basis of the quantitative and qualitative analysis,fromthree aspects of the development,management and evaluation of the path to build evaluation system,and a specialized subject hospital has carried on the empirical research,looking for the hospital clinical pathway from set up to implement the existing problems and defects in the evaluation process,analysis the reason and puts forward Suggestions,which will provide a scientific basis for its better standardize the clinical pathway,and then to the provincial hospital carrying out clinical pathway work provides experience for reference.Methods:1.Through a large amount of literature review,collecting documents and expert interviews,sorting out and summarizing the problems and experiences of clinical path management,and sorting out representative evaluation indicators.2.Apply the Delphi method to determine the index system.3.The hierarchy process was adopted to establish the weights of indicators at all levels.4.The field investigation method was adopted to make an empirical study on a specialized hospital,to find out the problems and defects,and put forward countermeasures and Suggestions.Results:1.The reliability of the experts is high: the recovery rate of the questionnaire is100%,and the positive coefficient is greater than 90%;The overall authority coefficient is 0.83 > 0.70;Experts are familiar with the problem.2.After three rounds of expert consultation,establishes the evaluation index system of clinical pathway,including the path of the path of development,management and evaluation of the effect of three first-level indicators,organizational security,development and design,information system and so on nine secondary indexes,reasonable organization system,multi-disciplinary participation,such as clinical pathway for 27 tertiary indicators.3.Through analytic hierarchy process,index weight was established,WA1=0.2886,WA2=0.3224,WA3=0.3890.4.Consistency check results: among 30 groups of expert evaluation,indicators have 28 group CR < 0.1,the secondary indicators have 27 CR < 0.1,level 3 indicators have 20 groups of CR < 0.1,the concentration of expert opinion.5.Through the empirical study,the overall score of the hospital was 62.07,and there were major defects in development and management,and the cost control and medical quality needed to be improved.Conclusion: the expert reliability is high,the coordination degree is high,the result is desirable,the index evaluation system constructed can provide guidance for clinical path management.A specialized hospital should strengthen the development and management of the route,strictly control the cost,reduce the waste of medical resources,and improve the medical quality of the hospital.Conclusion: 1.Expert reliability and authority are verified,and opinions are highlyconcentrated,so the indicator system is relatively scientific and objective.2.This system not only involves the establishment and implementation of clinical pathway,and contains the implementation of the results,the process control combined with the results can be achieved,but can be used as a scientific evaluation tools to provide specialized subject hospital clinical pathway management guidance.3.In the future,specialized hospitals should strengthen the development and management of routes,strictly control costs,reduce waste of medical resources and improve the quality of medical care. |
