| Objective:This research assessed the value of HPV E6/E7 mRNA texting in the cervical cancer Screening.It was aimed to discuss the relationship between HPV E6/E7 mRNA and cervical lesions,to analysis the sensitivity,specificity of HPV E6/E7 mRNA and liquid based cytology for high-grade lesions of the cervix,to teste HPV E6/E7 mRNA by cervical exfoliated cells and cervical paraffin-embedded tissue specimens.Methods:This study recruited 104 HPV-infected women with liquid based cytology,who experienced HPV E6/E7 mRNA and colposcopy cervical biopsy.Patients were grouped by pathological diagnosis criteria during the study.And HPV E6/E7 mRNA were analyzed by cervical exfoliated cells of all patients,60 of who were randomly use cervical paraffin-embedded tissue to teste HPV E6/E7 mRNA.Results:1.The positive rates of HPV E6/E7 mRNA in the three groups of cytology(NILM,ASCUS/LSILand HSIL)were different(P<0.05),and the positive rates were 60.00%,78.79%,90.32%,respectively,showing a gradual upward trend.Comparison of multiple recombinations found that the positive rates of NILM and HSIL were significantly different((χ~2=8.193,P~*<0.0125).The positive rates of ASCUS/LSIL andNILM,HSIL were not significantly different(χ~2=2.958,1.613,P~*<0.0125).2.The positive rates of HPV E6/E7 mRNA in the three groups of histological grades(cervicitis,low-grade CIN and high-grade CIN)were different(χ~2=11.636,P<0.05),and the positive rates were 59.10%,62.50%,90.00%,respectively,showing a gradual upward trend.Comparison of multiple recombinations found that the positive rate was different between the cervicitis group and the low-grade CIN group compared with the high-grade CIN(χ~2=9.318,8.979,P~*<0.0125).There was no difference in the positive rate between the cervicitis group and he low-grade CIN group(χ~2=0.064,P~*>0.0125).3.The Se,Sp,PV+and PV-of HPV E6/E7 mRNA were 90.00%,38.89%,57.69%,80.77%,respectively,and the corresponding of LCT were 88.00%,40.74%,57.89%,78.57%,respectively.There was no significant difference in sensitivity and specificity between the two diagnostic methods(P=0.227,0.09).The concordance rates of the two diagnostic methods was 78%(39/50)for biopsy-confirmed CINII+and was 57.41%(31/54)for biopsy-confirmed CINII-.4.Of 60 women scheduled for HPV E6/E7 mRNA texting,70%(42/60)women is positive in the cervical exfoliated cell group,65%(39/60)women is positive in cervical paraffin-embedded tissue group.Both exfoliated cervical cells and cervical paraffin-embedded tissue could be used to detect HPV E6/E7 mRNA(P=0.701).Conclusion:1.In the cytological level,the positive rate of HPV E6/E7 mRNA improved along with the severity of cervical cytological lesions.2.In the histological level,the positive rate of HPV E6/E7 mRNA improved along with the severity of cervical lesions.3.HPV E6/E7 mRNA detection can be used in triage of women with HR-HPV positive.4.The cervical paraffin-embedded tissue may be available to text HPV E6/E7 mRNA in cervical cancer prevention. |
