Analysis Of Increment Of Oxycodone Hydrochloride Sustained-release Tablets Between Related Factors Of Cancer Pain

Objective:Oxycodone hydrochloride sustained-release tablets can effectively relieve chronic moderate and severe pain in patients with advanced cancer,but the pain control is not good during the process of pain relief,so it is necessary to increase the analgesic dose.However,the incremental mechanism is not clear,and there is no research proof whether it is related to disease progression or drug-resistant addiction.In this study,we retrospectively analyzed the correlation of many factors affecting the increment of oxycodone hydrochloride sustained-release tablets,expounded the influencing factors of its increment,and provided the theoretical basis for the safety and effectiveness of the drug for the treatment of terminal cancer palliative therapy,to guide clinical treatment.Methods:1.The related data of 276 patients in The First Affiliated Hospital of Dalian Medical University with advanced cancer and chronic moderate and severe pain were collected.The characteristics of patients,tumor treatment and related factors of cancer pain treatment were analyzed.The correlation between the increment of oxycodone hydrochloride sustained-release tablets and the factors above was analyzed.2.Univariate and multivariate Logistic regression analysis was used to analyze the factors related to the increment of oxycodone hydrochloride sustained release tablets.The data included sex,age,tumor type and metastasis,radiotherapy,bisphosphates and other anti-tumor therapies,review of imaging and tumor markers,the NRS score,types of cancer pain,the dosage of oxycodone hydrochloride,the time of rediagnosis,the pain control situation,the dosage and dosage form of adjusting the drug.The difference was statistically significant when P< 0.05.Results: 1.Curative effect of cancer pain:The median score of NRS is 3.0 in 236(85.5%)patients with pain relief within 3 days.The median score of NRS is 2.0 in 260(94.2%)patients with pain relief within 7 days.The median NRS of the incrementers is 5.6 points,and the median NRS of 3 days after dosing is 2.9 points.2.Curative effect of tumor therapy: Imaging showed that 142 cases of PD were 51.4% and 118 cases of SD were42.8%,16 cases of CR / PR were 5.8%.3.Adverse reaction:In this study,the main adverse reactions after the addition of oxycodone hydrochloride sustained-release tablets were also constipation in 125 cases(45.3 %),lethargy in 3 cases(1.1 %),nausea in 2 cases(0.7 %),vomiting in 1 case(3.6 %).4.Factor analysis: Bone metastasis,severe pain of initial treatment and disease progression were the factors that caused the increment of oxycodone hydrochloride sustained-release tablets(P< 0.05).Sex,age,pain type,tumor type and visceral metastasis had no significant influence on its increment(P> 0.05).Radiotherapy,bisphosphonates and other anti-tumor treatments had no significant effect on its increment(P> 0.05).Conclusion:1.Oxycodone hydrochloride sustained-release tablets can control cancer pain.When cancer pain progresses,the increment of oxycodone hydrochloride sustained-release tablets can control the pain again.The pain relief rate was 85.5 % in 3days and 94.2 % in 7 days.2.Bone metastasis,severe pain of initial treatment and disease progression are the factors for the increment of oxycodone hydrochloride sustained-release tablets.3.There is no significant correlation between the increment of oxycodone hydrochloride sustained-release tablets and sex,age,types of cancer pain,tumor type,visceral metastasis.Radiotherapy,biphosphonate and other anti-tumor therapy were no significant correlationt with increment of oxycodone hydrochloride sustained release tables.4.The main adverse reactions of oxycodone hydrochloride sustained-release tablets after increment also are constipation,followed by drowsiness,nausea and vomiting,which can be tolerated.
