The Study On The Relationship Between The Distribution Of CAP TCM Syndrome Type And TCM Constitution

ObjectiveTo observe the characteristics of syndrome type,constitution characteristics and the relationship between constitution type and TCM syndrome type of patients with community-acquired pneumonia(CAP),so as to provide theoretical guidance for the prevention and treatment of pneumonia and to change the influence of constitution on the pathogenesis of the disease.MethodsThe basic information of patients diagnosed as CAP in Department of Respiratory Medicine in our hospital from May 2017 to March 2018 was collected through a questionnaire survey conducted by medical staff.According to the criteria of the Chinese Academy of TCM constitution classification and judgment,the patients were judged by the criteria.Then,the relationship between TCM syndromes and constitution of patients with CAP was preliminarily obtained by using SPSS19.0 statistical software to analyze the general situation of syndrome type and physique of patients with CAP.Results1.According to the diagnostic criteria,225 cases of CAP were collected after signing informed consent.There were 111 males and 114 females,the oldest being 83 years old and the youngest 18 years old.In terms of age distribution,the elderly group was the highest(38.2%),followed by the youth group(35.6%)and the middle age group(26.2%).2.The distribution of CAP syndromes was as follows: syndrome of phlegm-heat obstructing lung(89 cases),syndrome of wind-heat invading lung(45 cases),phlegm turbid obstruction of lung syndrome(27 cases),lung and spleen Qi deficiency syndrome(17 cases),external cold and internal heatsyndrome(14 cases),lung and stomach heat syndrome(11 cases),Qi and Yin deficiency syndrome(8 cases).3.Only one constitution has 84 people,two kinds of constitution have 133 people,three kinds of constitution and above have 8 people.There were 375 times of nine kinds of constitution,including 80 times of peaceful constitution,111 times of deficiency of qi,42 times of yang deficiency,20 times of yin deficiency,60 times of phlegm and dampness,33 times of dampness and heat,15 times of blood stasis,9 times of qi stagnation,5 times of special nature constitution.4.There was statistical significance in different age groups and inflammatory indexes of CAP.5.There were significant differences in age,sex and body size among patients with CAP.6.The type of constitution of CAP patients was not related to WBC and C-reactive protein,but correlated with procalcitonin.Conclusion1.The syndrome type of CAP patients is the most common in phlegm-heat obstructing lung and wind-heat invading lung syndrome.The syndrome of wind-heat invading lung is mostly seen in young people,Syndrome of deficiency of lung and spleen qi and qi-yin deficiency syndrome in the elderly.2.On the whole,the patients with CAP mainly have partial constitution and double clamp constitution.The most common constitution of CAP is qi deficiency,calmness,phlegm dampness and yang deficiency.Peace,moisture and heat are more common among young people,qi deficiency,phlegm and dampness are more common in old people.Dampness and heat are more common in men,Yin deficiency and qi stagnation are more common in women.3.There is a certain correlation between TCM syndrome type and constitution type of CAP.Most of the patients with mild,damp-heat and phlegm-dampness are characterized by phlegm-heat blockage of the lung.Mild constitution also tends to wind-heat invading the lung syndrome,phlegm-heat obstruction of the lung syndrome.Qi deficiency and yang deficiency are mostly due to deficiency of lung and spleen qi.Phlegm dampness constitution is more often manifested as phlegm turbid obstruction of the lung syndrome.Deficiency of yin and deficiency of qi are the main manifestations of deficiency of qi and yin.This is an organic combination of "syndrome differentiation" and "syndrome differentiation and treatment" in the treatment of CAP patients,and has a certain guiding and practical significance in improving the clinical efficacy.
