ObjectiveIn this clinical study, patients with coronary heart disease and anxiety-depression state were used as the observation objects. In order to study the clinical effect of treating coronary heary disease complicated with anxiety and depression state by Xinling Wan,to provide the clinical basis for the treatment of coronary heart disease with anxiety and depression state. To provide new ideas and methods for the treatment of Chinese medicine.MethodsIn this study,a randomized controlled trial method was used to collect 100 cases of patients,100 cases of final effective cases,of which 50 cases were treated group and 50 cases were in the control group.Chinese medicine symptom rating scale,Hamilton Anxiety Scale(HAMA),Hamilton Depression Scale(HAMD),Self-rating somatization symptom scale(SSS),Patient Health Questionnaire-9(PHQ-9),General Anxiety Disorder-7(GAD-7)and Seattle angina questionnaire(SAQ)were used as the observation indexes. In the two groups were basically treated with conventional treatment of coronary heart disease, the treatment group were treated with Xinling Wan.All treatment for a month, then compare the two group of TCM syndrome score Chinese medicine symptom rating scale,HAMA,HAMD,SSS,PHQ-9,GAD-7,SAQ changes and the incidence of adverse reactions were observed before and after the treatment.Results1.Baseline data analysis: There is no difference in gender, age, vascular lesion status, scores of TCM symptom score scale,SAQ score,HAMA score,HAMD score,SSS score,PHQ-9 and GAD-7 score of the two groups were compared. There was no significant difference in the distribution among the two groups(P>0.05).2.Two groups of TCM symptom scores analysis: The total effective rate was 46% in the treatment group and 22% in the control group. There was a statistically significant difference between the two groups(P<0.05).The total effective rate of the treatment group was higher. The therapeutic effect of TCM syndrome in the treatment group was better than that in the control group. After treatment, the scores of TCM symptoms in both groups were significantly lower than before treatment(P<0.01).The TCM symptom scores of the treatment group were lower than the control group, and the difference was statistically significant(P<0.05),and the difference between the two groups was statistically significant(P<0.01).3.Analysis of two groups of Hamilton Anxiety Scale(HAMA): The total effective rate was 62% in the treatment group and 44% in the control group. The difference between the two groups was statistically significant(P<0.05).The therapeutic effect of the treatment group was better than that of the control group. After treatment, the HAMA scores of both groups were lower than before treatment, and there was statistical significance(P<0.01).The HAMA score of the treatment group was lower than that of the control group, and there was significant difference(P<0.01),and the difference between the two groups was statistically significant(P<0.01).4.The Hamilton Depression Rating Scale(HAMD)analysis showed that the total effective rate was 60% in the treatment group and 40% in the control group. The difference between the two groups was statistically significant(P<0.05).The therapeutic effect of the treatment group was better than that of the control group. After treatment, the HAMD scores of both groups were lower than before treatment, and there was statistical significance(P<0.01).The HAMD score of the treatment group was lower than that of the control group and there was a significant difference(P<0.01),and the difference between the two groups was statistically significant(P<0.01).5.Two groups of Somatic Symptom Checklist(SSS)analysis: The total effective rate in the treatment group was 66%,and the total effective rate in the control group was 48%.The difference between the two groups was statistically significant(P<0.05).The therapeutic effect of the treatment group was better than that of the control group. After treatment, the SSS scores of both groups were lower than before treatment, and there was statistical significance(P<0.01).The SSS score of the treatment group was lower than that of the control group, and there was a significant difference(P<0.05),and the difference between the two groups was statistically significant(P<0.05).6.Two groups of PHQ-9 self-evaluation of depression(PHQ-9)analysis: The total effective rate in the treatment group was 62%,and the total effective rate in the control group was 42%.The total effective rate of the treatment group was higher, and the difference between the two groups was statistically significant(P<0.05).The therapeutic effect of the treatment group was better than that of the control group. After treatment, the PHQ-9 scores of both groups were lower than before treatment, and there was statistical significance(P<0.05).The PHQ-9 score of the treatment group was lower than that of the control group, and there was a significant difference(P<0.05),and the difference between the two groups was statistically significant(P<0.01).7.Two groups of GAD-7 anxiety self-assessment(GAD-7)analysis: The total effective rate in the treatment group was 68%,and the total effective rate in the control group was 46%.The total effective rate of the treatment group was higher and there was a statistical difference between the two groups(P<0.05).The therapeutic effect of the treatment group was better than that of the control group. After treatment, the GAD-7 scores of the two groups were lower than before treatment, and there was statistical significance(P<0.01).The GAD-7 score of the treatment group was lower than that of the control group, and there was a significant difference(P<0.05),and the difference between the two groups was statistically significant(P<0.01).8.Two groups of Seattle Angina(SAQ)scale analysis: after treatment, the degree of physical limitation, stable status of angina, angina pectoris, satisfaction with treatment, and disease awareness were improved compared with before treatment(P<0.05).Xinling Wan group was better. The scores and differences between the two groups were not statistically different(P>0.05).9.Safety testing and adverse reaction evaluation: During the clinical trial,no adverse reactions occurred in the treatment group and the control group during the treatment. Conclusion 1.Xinling Wan can effectively improve TCM symptom scores in patients with coronary heart disease and anxiety and depression(P<0.05).2.Xinling Wan can improve the scores of HAMA,HAMD,SSS,PHQ-9,and GAD-9 in patients with coronary heart disease and anxiety and depression, and have a certain effect in improving anxiety and depression in patients(P<0.05).3.Xinling Wan can improve the score of Seattle Angina Pectoris in patients with coronary heart disease and anxiety and depression(P<0.05),but it can not be considered that there is a significant difference in the effect of Pill Pill between the control group and the control group(P>0.05).4.Xinling Wan has higher safety, less adverse reactions.
