ObjectiveBy observing the clinical curative effect of Qing Sui Xiao Yan Decoction combined with antibiotics in the treatment of chronic osteomyelitis(heat-toxin congestion and excessiveness),to evaluate the improvement and advancement of the anti inflammatory and anti-inflammatory,the wound surface,the clinical effect and the recurrence rate,and evaluate the safety of the decoction.MethodsOf the 60 cases who were admitted to the Orthopedics Department of Henan Provincial Hospital of Traditional Chinese Medicine during 2017.3 to 2017.10,49 were male and 11 were female.According to the different treatment methods,random number grouping was used to divide the control group and the experimental groups.The experimental group consisted of 24 males and 6 females,aged 42.20±9.98 years.The disease course was ≤6 months in 16 cases,6-12 months in 10 cases,and ≥12 months in 4 cases.The diseased area included 1 humerus,9 femurs,and 18 patellas,calcaneus in 2 cases.The control group consisted of 25 males and 5 females,aged 42.50±10.15 years,17 patients less than 6 months of disease,6-12 months in 10 cases,and ≥12 months in 4 cases,including 2 with humerus,7 femurs,and 20 with tibia,calcaneus in 1 cases.In the treatment group,the control group was treated routinely.The treatment was first debridement,removal of foreign body and purulent secretions in the lesion,removal of inflammatory granulations and scars in the sinus tract and lesions,alternating irrigation with saline and hydrogen peroxide,diluting iodine soaking for 15 minutes,re-sterilize the drape,visually close the wound during the operation,unable to close the wound dressing aseptic filling,regular dressing,postoperative drug sensitivity test and bacterial culture as a reference,select the appropriate sensitive antibiotics intravenous drip for 2 weeks,and then changed to oral 2 weeks.The experimental group was treated with Qing Sui Xiao Yan Decoction as the main prescription for oral treatment for 4 weeks.Observe the changes in the anti-infective efficacy indicators,bacterial changes,and wound recovery in the two groups.Compare the efficacy and recurrence rates of the two groups of cases,evaluate their safety,and record adverse events.Results1.Comparison of anti-infective efficacy indicators,preoperative and postoperative 1 week,P>0.05,no statistically significant,and there was no significant difference in reduction of anti-infective indicators between the two groups of patients,indicating that before treatment and 1 week after surgery,there were no significant differences in the control of inflammation in the group of patients taking different treatments.At 2 weeks and 4 weeks postoperatively,P<0.05 was statistically significant,and the index of inflammation in the experimental group was significantly lower than that in the control group,indicating that the experimental group had more superior curative effect in controlling inflammation than the control group.After repeated measurement and analysis of variance,regardless of grouping effect,P<0.05,indicating that all patients’ treatment effects changed correspondingly with different time periods.The P<0.05 between group and time effects showed that there was an interaction between time and group,and as time changed(ie,1 week,2 weeks,and 4 weeks after surgery),the effect of the test group and the control group in controlling inflammation was changes in time have undergone different changes.2.Comparison of bacterial culture changes before treatment,1 week after surgery,P>0.05,no statistical significance.After 2 weeks of treatment,the P<0.05 test was statistically significant.The results showed that there was no significant difference in antibacterial activity between the experimental group and the control group.After 2 weeks,the cases who received oral Qing Sui Xiao Yan Decoction had a better antibacterial effect than the patients with a single treatment method.After two weeks of treatment,the two groups of cases were compared with those before treatment,the p were all less than 0.05,which was statistically significant,indicating that both therapies had bacteriostasis.3.Safety test and adverse event analysis.Liver function and renal function were examined before and after treatment,and changes in AST,ALT,and Cr were observed.Differences in AST,ALT,and Cr between the two groups before and after treatment were compared within and between groups.After testing,P were all greater than 0.05,with no statistical significance,and no serious or significant adverse reactions occurred during the observation period,suggesting that the Chinese medicine has good safety,with little side effects,it can be used for a long time.4.Efficacy evaluation indicators: After the treatment,the experimental group and the control group were effective after treatment.After the test,P<0.05,there was a statistical significance,and the experimental group effective rate was 93.33%,while the control group was 76.67%,suggesting that the combined Qing Sui Xiao Yan Decoction can achieve better clinical efficacy than conventional treatment.5.The improvement of wound infection was tested,P<0.05,statistically significant,and the number of cases of improvement in wounds in the experimental group was significantly higher than that in the control group,suggesting that Qing Sui Xiao Yan Decoction has a better effect in improving wound infection.6.The scores of symptoms and signs before and after treatment were compared between the two groups of cases.Before and after treatment,the scores of symptoms and signs before and after treatment in the experimental group and the control group were compared at P<0.05,which was statistically significant.Compared between the experimental group and the control group,P>0.05 before treatment,there was no statistical difference,P<0.05 after treatment,there was a statistical significance,suggesting that oral Qing Sui Xiao Yan Decoction is better than single traditional therapy in improving symptoms and signs Efficacy.7.After follow-up for six months,the recurrence rate was 3.33% in the experimental group and 3.73% in the control group,P>0.05,which was not statistically significant,indicating that there was no significant difference in the long-term recurrence rate of the two different therapies.Conclusion:1.The use of Qing Sui Xiao Yan Decoction combined with antibiotics in the study group was more effective than single antibiotics in the control group in terms of antibacterial and anti-infection.2.Qing Sui Xiao Yan Decoction can enhance the patient’s physique,improve the symptoms,improve the body’s immunity,and help the recovery of the disease.3.Qing Sui Xiao Yan Decoction can improve the clinical curative effect in the early stage,but compared with the long-term recurrence rate;there is no significant difference between the two groups.4.Qing Sui Xiao Yan Decoction adopts granule preparation,which is convenient to take,safe and effective in clinical observation,and has no obvious toxic and side effects.It can be taken long-term.
