| OBJECTIVE:To observe the effect of ERBT on pathological diagnosis,and compare the effect of different surgical procedures and different energy platforms on pathological diagnosis after transurethral resection of bladder tumors,and to search for surgical procedures and energy platforms that are most conducive to pathological diagnosis.METHODS:Specimens of patients undergoing transurethral resection of bladder tumors at the Second Hospital of Tianjin Medical University from 2016 to 2017 were collected,and theses patients were diagnosed as NMIBC and confirmed by pathology.Among them,40 patients were treated with conventional resection group and 40 patients were enrolled in the en bloc resection group.Two pathologists used a micrometer to measure the eschar thickness of the specimen margin.The en bloc resection group was divided into plasma-energy platform and laser-energy platform,and observed the thickness of the basement and surgical margins and the distance from the normal specimen edge to the tumor.RESULTS:We have collected 80 cases of TUR surgery,including 40 cases of conventional TUR group and 40 cases of en bloc resection group.15 cases were divided into plasma platforms according to different energy platforms among the en bloc resection group.Laser-energy platform included 25 cases.The histological integrity of the en bloc resection group was better than that of the traditional TUR group.The detection rate of detrusor was higher(100% vs 70%,χ2=14.11,p<0.05).The eschar of the margin in traditional TUR group(0.85±0.28)mm was smaller than that of the en bloc resection group(1.22±0.27)mm(P<0.05).The laser-treated specimen had a larger eschar of basement(1.33±0.25)mm than plasma group(1.03±0.22)mm(p<0.05).The eschar of the edge in laser group was(0.86±0.15)mm,which was larger than plasma group(0.75±0.14)mm(p<0.05),the eschar of the laser group mainly occurs in the superficial muscular layer and does not reduce the pathological stage.CONCLUSION:ERBT has showed more advantages than conventional TURT in the pathological diagnosis of specimens,and a relatively more continuous hierarchy and a more complete organizational structure can be obtained after En bloc surgery.The laser-energy platform will produce a more thick eschar compared with the plasma,but it mainly occurs in part of the muscle layer,and has no obvious effect on the pathological staging. |
