ObjectiveThe osteonecrosis of the femoral head is a common and refractory disease,generally divided into traumatic and no-traumatic.The final result is by the different reasons undermines the blood supply of the femoral head.The disease incidence is higher in young adults,course of disease rapid progression.If treatment is not timely,most patients with unfavourable prognosis.Due to the economic and social burden.For middle-age and young patients,the standard appendicitis is save the femoral head.At present there is no method of exact curative,a variety of methods can improve the curative effect.With the progress of clinical and foundation medicine in recent years,the treatment of the disease have further research.This research used the renewal and multiply ability of bone marrow mesenchymal stem cells,treatment of osteonecrosis of the femoral head is by core decompression and transplantation bone marrow mesenchymal stem cells with porous hydroxylapatite composite.To explore a new method for treatment of ONFH.MethodsThree parts.Part 1: establish the dog femoral head osteonecrosis models by the modified of liquid nitrogen.The models on side are established in 12~18 month dog using liquid nitrogen.Cut the hip joint capsule,prolapsed the femoral head.A chipless polyfoam protect surrounding soft tissue,the funnel cover the femoral head,to inject liquid nitrogen into funnel hold 3min,37℃ Warm saline rewarming the femoral head and reduction.Sutured layer by layer.All dogs are unilateral operation,thecontralateral was treated as control.The radiological and histology examination are made at the 6 week,12 week after the models have been established.Part 2: to isolate and cultivate,preserve and identify the dog bone marrow mesenchymal stem cells.extracting ilium bone marrow by Ilium puncture at12~18 month dog,then isolate bone marrow mesenchymal stem cells using density gradient centrifugation method and cultivate and purified cell by the attachment culture method.Futher,expanded cells,by measuring surface antigen expressiong of identification.Part 3: the research about core decompression and transplantation bone marrow mesenchymal stem cells with porous hydroxylapatite composite in treatment of osteonecrosis of the femoral head.The osteonecrosis of the femoral head models on one side are established in 12~18 month dog using liquid nitrogen.Randomly classified into three groups after models have been made 12 weeks.Group A are conducted core decompression and transplantation bone marrow mesenchymal stem cells with porous hydroxylapatite composite.Group B are conducted core decompression.Group C are conducted core decompression and transplantation bone marrow mesenchymal stem cells 。 The health side of three groups is treated as self-control.The radiological and histology examination examination are made at 3,6 months after the bone grafting.ConclusionsThe way of core decompression and transplantation bone marrow mesenchymal stem cells with porous hydroxylapatite composite may promote the repairment of femoral-head osteonecrosis.
