Objective: To investigate the correlation between serum hydroxyvitamin D level and fragility hip fracture.Methods: The clinical data of 181 female fragility hip fracture patients(age ≥50 years)were retrospectively analyzed,who were admitted to our hospital from January 2013 to December 2015 and underwent serum 25-hydroxyvitamin D(25OHD)test.The age,fracture site,serum 25 OHD level and bone mineral density(BMD)of patients,as well as the outcomes of 67 patients followed up for at least one year were recorded.Based on age,patients were assigned to ≤75 years group and >75 years group to observe the correlation between age and serum 25 OHD level.Based on fracture site,patients were assigned to femoral neck fracture group and intertrochanteric fracture group to observe the correlation between fracture site and serum 25 OHD level.Based on 25 OHD level,patients were assigned to 25OHD<20ng/ml group and 25OHD≥20ng/ml group to observe the correlation of serum 25 OHD level with BMD and patient outcome.Results: The mean age of 181 female fragility hip fracture patients(age ≥50 years)was 80.12±8.13 years,and the average length of stay was 15.2±7.1 days.Among the patients,vitamin D was deficient in 127 cases(70.2%),insufficient in 42 cases(23.2%)and sufficient in 12 cases(6.6%).The serum 25 OHD level of patients ≤75 years was higher than that of patients >75 years,and the difference was statistically significant(p=0.016).The serum 25 OHD level of femoral neck fracture patients was higher than that of intertrochanteric fracture patients,but the difference was not statistically significant.The femoral neck BMD and lumbar BMD of 25OHD≥20ng/ml patients were both higher than those of 25 OHD < 20ng/ml patients,and the differences were both statistically significant(p < 0.05).Among patients followed up,25OHD≥20ng/ml patients had lower fall rate,fracture rate and all-cause mortality than 25OHD<20ng/ml patients within one year,but the differences were not statistically significant.The outdoor action ability of 25OHD≥20ng/ml patients was stronger than that of 25OHD<20ng/ml patients,and the difference was statistically significant(p=0.028).Conclusion: Vitamin D deficiency and insufficiency are prevalent in elderly femalepatients with fragility hip fracture.Serum 25 OHD level declines gradually with age.Therefore,raising serum 25 OHD level can increase femoral neck and lumbar BMD,as well as improve the outcome of elderly female patients with fragility hip fracture.In these patients,higher serum 25 OHD level is associated with stronger outdoor action ability.
