Object To analyze the factors in the pathogenesis of Polycystic Ovary Syndrome(PCOS)that may cause changes of the patient’s Bone mineral density(BMD),and to explore the effect of low frequency electrical technology combined with Diane-35 on BMD and its influence factors in PCOS patients.Method Based on the International Rotterdam Diagnostic Criteria in 2003,70 patients who were diagnosed with PCOS for the first time from September 2016 to October 2017 were selected as subjects and were aged 18-35 years.BMD of lumbar spine(L1-L4)and femoral neck,bone metabolic indexes(TPINP,β-CTx,BGP),sex hormone(E2,FSH,LH,T),glucose metabolism indexes FBG,FINS,and HOMA-IR and obesity indexes BMI,WHR were measured before treatment.Part 1: The patients were divided into normal bone density group and reduced bone density group according to the reduction in BMD.The differences in E2,FSH,LH,T,FBG,FINS,BMI,WHR,and HOMA-IR were compared between the two groups of PCOS patients.Screening factors which may affect BMD changes in PCOS.Part2: According to the patient’s therapy willingness,they were divided into trial group(low frequency electrical stimulation +Diane-35)and control group(Diane-35)for 3 cycles therapeutic intervention.The differences in the influence factors of BMD,TPINP,β-CTx,BGP,and BMD were compared between the two groups after treatment,Result Part1: The factors whcih could influence the BMD were compared between the reduced bone group and the normal group :The BMI in reduced bone group before treatment was lower than that in patients with normal bone group(P<0.05),and the WHR,FINS,LH,and T were higher than those in the normal bone group(P<0.05).There was no Significant difference in age,E2,FSH,and FBG of both groups(P>0.05);Part2: There was no significant difference in age between the trial group and the control group before treatment(P>0.05).After 3 cycles of treatment,the lumbar(L1-4)BMD of the two groups were higher than before(P<0.05),BMD of the femoral neck was not significantly changed(P>0.05);The expression of bone metabolic markers TPINP,β-CTX and BGP decreased in both groups after treatment(P<0.05).Comparison of efficacy between groups: The improvement scores of BMD,TPINP,β-CTX and BGP were better in the trial group than in the control group(P<0.05).Analyzed and comparied the Influencing Factors of BMD After Treatment:In trial group :the BMI and WHR were lower after treatment,The glucose metabolism indicators FINS and HOMA-IR decreased than before(P<0.05),the levels of sex hormones LH and T decreased(P<0.05);In control group:the obesity indicators BMI and WHR increase slightly(P>0.05),FINS,HOMA-IR and sex hormones LH and T were lower than before(P<0.05).Comparison of efficacy between two groups: the effects of BMI,WHR,FINS,HOMA-IR,LH and T were better improved in the trial group than in the control group(P<0.05).Conclusion 1.Central obesity,IR and hyperandrogen may have negative effects on bone quality in patients with PCOS;2.low frequency electrical technology combined with Diane-35 can effectively improve bone metabolism and increase BMD in patients with PCOS,and the overall effect is better than that of Diane-35.
