| ObjectiveTo construct a scientific and applicable evaluation index system for the post competence of nurses in the emergency department in China,and to provide a theoretical basis for the recruitment,training and assessment of nurses in the emergency department.MethodsThrough literature review and behavioral event interview,the competency theory framework of nurses in emergency department was constructed.Two rounds of expert consultation method was used to modify and improve the initially established indicators to form the competency evaluation index system for nurses in the emergency department.Then the analytic hierarchy process is used to determine the weight coefficient of the first and second level indexes.Finally,the convenience sampling method was used to conduct a pre-survey of 137 nurses in the emergency department of5 comprehensive third-level first-class hospitals in hubei province through the competency self-assessment questionnaire,to determine the reliability and validity of the questionnaire and further verify the inherent quality of the indicator system.Results(1)The evaluation index system of the competence of nurses in the emergency department is constructed,which consists of four first-level indicators including professional knowledge and skills,professional ability,personal characteristics and motivation,and 44 second-level indicators.(2)The weighted professional knowledge and skills(0.2136),professional ability(0.3253),personal characteristics(0.2136)and motivation(0.2474)of the four first-level indicators were Professional knowledge and skills dimension including the basic theory knowledge(0.0743),(0.0503),the specialized nursing emergency theories technology of intensive care patients(0.0454),emergency common drugs(0.0503),emergency green channel knowledge(0.0650),the common emergency contingency plans(0.0744),the pre-hospital first aid knowledge(0.0744)(0.0584),infectious diseases,emergency first aid knowledge(0.0612),pre clinical triage process(0.0741),nursing law(0.0690),emergency ethics related knowledge(0.0728),basic nursing operation technology(0.0614),emergency specialized nursing operation technology(0.0563),use and maintenance of emergency equipment and equipment(0.0454),and safe transport of critically ill patients(0.0674)16 indicators;Professional abilitydimensions including assessment and judgement(0.0757),illness observation(0.0757),and ability of comprehensive analysis ability(0.0478)(0.0757),emergency rescue,ability to communicate(0.0838),team cooperation ability(0.0811),humanistic care ability,ability to anticipate(0.1007)(0.1203),as a whole(0.1121),the scientific research ability(0.1153),teaching ability 11 index(0.1117);Personality dimensions include emergency consciousness(0.0517)(0.0517),team consciousness,self-supervision spirit(0.0002),responsibility(0.0002),responsive(0.0517),the concept of time(0.0002),high efficiency(0.0974),the careful(0.1243),the hard work(0.1243)(0.1279),service consciousness,empathy(0.1483)(0.1248),psychological quality,body quality 13 indicators(0.0975);Personality motivation includes four indicators: stress management(0.2759),personal development(0.2675),autonomous learning(0.2321),and self-reflection(0.2244).(3)After two rounds of expert consultation,the recognition rate of the indicators by experts ranged from 85 to 100%,the mean of the evaluation index ranged from 3.950 to5.000 points,and the coefficient of variation ranged from 0 to 15.634%,all reaching the standard.The effective questionnaire recovery rates of the two rounds of expert consultation are 92.592% and 100% respectively,and the average coefficient of expert authority is 0.93,indicating that the experts are highly active and authoritative,and the obtained evaluation index results are highly reliable.The coordination coefficient of the first-level index was 0.150,and the P value was 0.029(P < 0.05),and the coordination coefficient of the second-level index was 0.274,and the P value was 0.000(P < 0.05),indicating that the experts had a good degree of coordination and a high degree of agreement on the indicators.(4)The evaluation index system constructed in this study has good reliability and validity.The overall internal consistency reliability coefficient of the evaluation index system is 0.964,The internal consistency reliability coefficient of the first-level index is between 0.903 and 0.979,were greater than 0.8.Content validity was analyzed through expert consultation results,The variation coefficient of the index ranged from 0 to15.634% are less than 20%.Structural validity was analyzed by correlation,The correlation coefficient between the primary index and the index population was between0.671 and 0.791,The correlation coefficient between secondary index and primary index was between 0.172 and 0.909,P < 0.05.Conclusions(1)In this study,literature review,behavioral event interview,two rounds of expert consultation and relevant statistical analysis methods were used to construct thecompetency evaluation index system for nurses in the emergency department.It provides a theoretical basis for the recruitment,training and assessment of emergency department nurses,and is of great significance for the selection and training of excellent emergency department nurses.(2)The analytic hierarchy process(ahp)was used to set the weight coefficient of the indexes at all levels to reflect the relative importance of the indexes at all levels in the overall evaluation,and the indexes at all levels were evaluated objectively.(3)The positive coefficient,authoritative coefficient and index reliability and validity of the consulting experts in this study are relatively high. |
